社区医疗服务政府治理的期权模式

摘　　要：探索治理语境下的社区医疗服务政府治理模式具有重要的理论和现实意义。针对政府主导社区医疗服务存在的问题和困境,对政府主导社区医疗服务提出质疑。通过文献梳理和实践调研,提出以政府治理取代政府主导可能是解决问题的一种思路。尝试性地引入社区医疗服务多元治理和元治理理念,提出政府治理社区医疗服务的期权模式。利用弗里德曼提出的教育券模式设计一个以居民为持有者,社区卫生机构为出售者的社区医疗服务期权,通过政府的元治理使具有共同目标的多元治理主体共同实现保障居民基本医疗服务的责任。将期权定价模型引入社区医疗服务期权费定价,利用中国卫生统计年鉴数据测算按人头支付的期权费,为覆盖城乡居民的基本医疗卫生服务制度建设提供理论参考与决策支持。Exploring governance models of community health services in the context of governance has important theoretical and practical significance.For the problems and difficulties of government-led community medical,the paper questioned on government-led community medical service,and proposed to replace the government-led to governance may be a new idea to the problem through literature and practice research.Attempts to introduce multiple governance and meta-governance philosophy for community medical service,the option governance model of community health services were raised.Take advantage of Friedman＇s school voucher mode to design community health care services options with residents as owners and community health agencies as sellers to realize the common objectives of multiple governance body to achieve protection of residents＇ basic medical services through meta-governance.Take advantage of the option pricing model to measure community medical service option fee,to provide a theoretical reference and decision support for covering the urban and rural residents of basic health services system construction.
