摘 要:目的对2009年发生在安徽省境内的2起霍乱疫情进行溯源及2起疫情菌株的遗传相关性分析,为防控霍乱提供依据。方法应用血清学及常规细菌学方法对菌株进行鉴定,聚合酶链式反应(PCR)方法检测霍乱毒力基因,脉冲场凝胶电泳技术(PFGE)对霍乱菌株进行分子分型。结果所分离3株菌均是霍乱弧菌O139且具有肠毒素(CT)和毒素共调菌毛(TCP)毒力基因;PFGE分成2个型。SZ分离株的pattern在福建、广东、湖南、江浙和重庆出现过。该pattern的菌株在霍乱数据库占已有O139菌株的12.7%;。H1、H2的pattern与辽宁分离株具有相同的pattern。该pattern的菌株在霍乱数据库中目前仅有1株。结论 3株试验菌株均是产毒株,具有致病性;首例病人分离株与该病人所食甲鱼分离株PFGE带型一致,分子流行病学分析,甲鱼可能是首起疫情的传染源之一,与第二起疫情无相关性。Objective To trace the source of two cholera in Anhui province in 2009,analyze the genetic correlation between the bacterial strains isolated from the two cholera outbreaks,and provide a theoretical basis for cholera prevention and control.Methods The bacterial strains were identified by using the serological method and the conventional bacteriologic method,and were used to test for the virulence gene of cholera by PCR,and were also performed molecular subtyping of Vibrio cholera by Pulsed-field gel electrophoresis(PFGE).Results Three isolated strains were identified as vibrio cholera O139,and had cholera toxin gene and toxin coregulated pili gene.The strains were divided into two types by PFGE.The pattern of SZ field-isolates occurred in Fujian,Guangdong,Hunan,Jiangsu,Zhejian provinces and Chongqing city,and the strains of this pattern have accounted for 12.7% of O139 strains in the database of cholera.The pattern of HI and H2 was the same as the field-isolates in Liaoning,and there was only one strain in the database of cholera at present.Conclusion Three Strains were all toxigenic Strain,and had pathogenicity.The isolated strain of first case had the same type with the strain isolated from the soft-shelled turtle eaten by him,so the soft-shelled turtle may be one of sources of infection,and was not correlated with the second outbreak from the perspectives of molecular epidemiology.
