跗外侧血管蒂骰骨瓣移位术的临床应用

摘　　要：目的为了寻求和解决踝足部骨缺损、骨不连、距骨骨折脱位和距骨体缺血性坏死等疑难病例的有效疗法。方法通过对３０侧下肢标本解剖观测了供应骰骨背侧血管的来源、走行、分支和吻合。设计以跗外侧血管为蒂的骰骨瓣移位植骨术治疗有关疾患共２９例。结果临床实践证实疗效满意。有２７例经１～８年随访，其中优良率达９６％。结论带跗外侧血管骰骨瓣具有解剖位置表浅、手术操作简便、效果可靠等优点。Objectives To solve the problems of bone defect or non union of ankel and foot,fracture of talus neck or dislocation of talus body To improve the fusion of joint between tibia and talus or joint under talus Methods The origin,course,branch and anastomosis of vessels of dorsal cuboid bone were observed on 30 side lower limbe specimen Transposition of cuboid bone flap pedicled with lateral tarsal artery was designed for treatment of the diseases (29 cases) Results The therapeutic results were satisfactory Th period of follow up in 27 cases is one to eight years The excellent and good rate is 96 percent Conclusions Anatomical location of cuboid bone flap pedicled with lateral tarsal artery is superficial The operation is simple and the results is reliable There is a longer range of rotation This new bone flap is a good donor for repairing diseases of ankel and foot
