摘　　要：目的：总结应用主动脉根部-右房分流技术减少主动脉根部替换术后出血的临床效果和分流手术的技术难点与要点.方法： 回顾我院2004年2月至2010年2月的25例主动脉根部替换术,术中发生根部出血或预防性应用主动脉根部-右房分流18例.总结体外循环时间,术后胸腔引流量,平均输入血液制品量,二次开胸止血比率及出血相关并发症的发生率.结果：在18例主动脉根部-右房分流中男性12例,女性6例 平均年龄（40＋13.2）岁,体重（51-86）kg,平均（62＋11）kg.在完成分流后,所有患者主动脉根部均得到充分的止血,术后胸腔引流量400-1340ml,平均（848＋271）ml.1例术后4个月因移植血管感染死亡,1例2年后出现腹主动脉瘤破裂死亡外,余患者均存活.出院前常规超声心动图提示主动脉根部与右房间无血流流动.16排CT检查显示主动脉根部有血栓形成.结论：主动脉根部-右房分流技术操作简单,可以有效减少主动脉根部替换术后的出血,显著改善主动脉根部替换手术的临床效果,减少出血相关并发症,缩短手术时间,降低手术的费用.Objective：After the summary application aorta root -- atrium dextrum divergence technology reduction aorta root replace technique bleeds clinical effect and divergence surgery technical difficulty and main point. Methods： Reviews my courtyard from February, 2004 to February, 2010 25 example aorta root replace technique,in the technique has the root hemorrhage or preventive diverges 18 examples using the aorta root -- a- trium dextrum. Summarizes the external circulation time, after the technique the chest cavity drainage quantity, the average input blood product quantity, two times opens the chest hematischesis ratio and the hemorrhage correlation complication formation rate. Results：In 18 example aorta root -- a- trium dextrum divergence masculine 12 examples,feminine 6 examples;Average age （40＋13.2） year old,body weight （51--86）kg,average （62＋11） kg. After completes the divergence,all patient aorta root obtains the full hematischesis,after the technique the chest cavity drainage measures 400-- 1340ml,average （848＋271）ml. Because 1 example technique latter 4 months transplant the blood vessel to infect the death,after I example 2 years appear outside the abdomen driving block breakage death, --odd patient survives. Before out of hospital between conventional echocardiogram prompt aorta root and atrium dextrum non--blood stream flowing. 16 row of CT inspection demonstration aorta root has the thrombus to form. Conclusion： The aorta root -- atrium dextrum divergenee technical operation is simple, after may reduce the aorta root replace technique effectively the hemor- rhage,obviously improves the aorta root replace surgery the clinical effect, reduces the hemorrhage correlation complication, reduces the surgery time, reduces the surgery the expense.
