胃肠道异位胰腺60例的诊断与治疗分析

摘　　要：目的探讨胃肠道异位胰腺的临床特点、诊断及治疗原则。方法对近10年来经内镜超声检查和病理检查确诊的60例异位胰腺患者的病例资料进行回顾性分析。结果本组60例，病变位于十二指肠球腔3例，十二指肠降段2例，胃角3例，胃窦、体交界处2例，胃窦部50例。所有病变均经过内镜、内镜超声及病理检查确诊为异位胰腺。其中28例行内镜下高频电切治疗，2例行内镜下黏膜切除术（EMR）治疗，30例行外科手术治疗。所有患者术后疗效满意，未出现手术并发症。结论内镜超声对确诊异位胰腺具有重要价值，对于异位胰腺的内镜下治疗具有重要的指导意义。内镜下微创治疗异位胰腺安全有效，具有重要的临床应用价值。Objective To investigate the clinical features, diagnosis and treatment of heterotopic pancreas in gastrointestinal tract. Methods A total of 60 cases of heterotopic pancreas in gastrointestinal tract diagnosed through endoscopic ultrasonography and pathological examination were treated in our hospital in the recent 10 years. Their data were retrospectively analyzed. Results Among all the 60 patients, the lesions were located in duodenal bulb cavity in 3 cases, in duodenal descendent in 2 cases, in gastric angle in 3 cases, in the junction of gastric antrum and corpus ventriculi in 2 cases and in gastric antrum in 50 cases. All of the 60 patients were definitely diagnosed through endoscopy, endoscopic ultrasonography and pathological examination. 28 patients were treated by endoscopic high-frequency electronic resection, 2 patients were treated by endoscopic mucosal resection （EMR）, 30 patients were treated by surgical operation; no operative complication was found, and satisfactory clinical effects had been observed in these patients. Conclusions Endoscopic uhrasonography has great value in the diagnosis and endoscopic therapy of heterotopic pancreas in gastrointestinal tract. It can provide the guide for the choice of treatment. Minimally invasive endoscopic treatment is safe and effective for heterotopic pancreas and has great clinical value.
