16层螺旋CT血管成像技术在闭合性血管损伤中的应用

摘　　要：目的 探讨16层螺旋CT血管成像技术（16-SCTA）在诊断闭合性血管损伤的应用价值.方法 对54 例可疑有动脉血管损伤的病人进行16-SCTA,利用Smart prep跟踪技术.结果 全部CTA检查均获得成功.CTA显示动脉闭塞26条、血管内充盈缺损4条、造影剂外渗13条、动脉壁粗糙不光滑7条、动脉狭窄14条;其中动静脉瘘4例、假性动脉瘤2例,动脉内造影剂排空延迟4例.手术治疗52例：动脉完全断裂17例、不完全断裂26例,内膜损伤11例、合并间隔综合征4例;端端吻合27例,直接修补9例,静脉移植16例.介入性栓塞2例.术后受损动脉血流通畅、血运正常.栓塞2例出血停止,无循环及功能异常.结论 16-SCTA是诊断血管损伤方法之一,并可对治疗有指导性作用.Objective To investigate the value of 16-slice spiral computed tomography angiography（16-SCTA） in the closed vessels injury. Methods 16-SCTA were performed in 54： patients who were suspected with vascular injury,and Smart prep tracking technology was used at the same time. Results CTA in all cases was done successful. CTA showed arterial occlusive disease in 26 vessels,vascular filling defect in 4, extravasation of contrast agent in 13, unsmooth of arterial wall in 7 and arterial stenosis in 14. There were arteriovenous fistula in 4： cases, pseudoaneurysm aneurysm in 2 cases and intra--arterial contrast agent delayed empty in 4 cases. 52 cases underwent surgical treatment, including arterial rupture completely in 17 and partly in 26 cases, endometrial injury in 11 cases,in combination with compartment syndrome in 4 cases. In 5Z cases, 27 cases underwent end--to-end anastomosis, direst repair in 9 cases and vein graft in 16 cases,the other 2 cases underwent interventional embolization. Blood flow was free and normal after operation in all cases. 2 cases after embolization,bleeding stopped and no abnormality of circulation and function. Conclusion 16 -SCTA is one of the methods in diagnosis of vascular injury,and is of guiding role for clinical therapy.
