摘 要:目的探讨胚胎后肾间充质干细胞对多柔比星肾病(ADN)模型(微小病变型)大鼠足细胞Nephrin、Podocin表达的影响。方法将大鼠随机分为模型组、胚胎后肾间充质干细胞注射组(RIMM-18细胞组)和对照组,各18只。模型组、RIMM-18细胞组大鼠一次性尾静脉注射多柔比星5 mg.kg-1,对照组予尾静脉注射9 g.L-1盐水3 mL。造模3 d后RIMM-18细胞组及对照组分别予尾静脉注射1×107L-1RIMM-18细胞和1 mL无血清培养液。造模第6、17、31天检测各组大鼠24 h尿蛋白、血清清蛋白、血清胆固醇及肾脏病理改变,采用免疫荧光及Western blot检测各组大鼠Nephrin、Podocin表达情况。结果在造模第31天RIMM-18细胞组大鼠24 h尿蛋白及血清胆固醇较模型组明显减少,血清清蛋白明显升高,足突融合情况明显减轻,足细胞Nephrin、Podocin表达明显增多(Pa<0.05)。结论尾静脉注射胚胎后肾间充质干细胞可减少ADN大鼠的蛋白尿,减轻ADN大鼠肾组织足细胞足突融合。其可能的机制是通过上调Podocin、Nephrin蛋白的表达从而维持蛋白复合体的结构和功能的完整。Objective To investigate the effect of embryonic kidney mesenchymal stem cells on the expression of Nephrin and Podocin in the model rats with adriamycin-induced nephropathy(ADN)(minimal-change type).Methods The rats were divided into model group(n=18),control group(n=18) and rat-inducible metanephric mesenchymal-18(RIMM-18) group(n=18).Model group and RIMM-18 group were injected adriamycin(5 mg·kg-1) only once via caudal vein.At the same time,the rats in the control group were injected 9 g·L-1 saline(3 mL).After 3 days,the rats of control group were given culture medium via caudal vein(1 mL) and those of RIMM-18 group were injected RIMM-18 cells(1×107 L-1).On the 6 days,17 days and 31 days after the models being established,the urine and blood from the rats were collected to examine urine protein in 24 hours,serum albumin and serum cholesterol.The kidneys from the rats were all collected to examine pathological change of renal tissue and observe the expression of Nephrin and Podocin which were marker proteins of podocyte with the technique of immunofluorescence and Western blot.Results On the 31 days after the model being manufactured,the 24 hours urine protein and serum cholesterol in RIMM-18 group were lower than those in the model group.At the same time,the increase of serum albumin,alleviation of foot process fusion and the growth of the expression of Nephrin and Podocin were obviously at all(Pa0.05).Conclusions RIMM-18 may decrease urine protein in ADN rats,alleviate foot process fusion,which might be noted through the up-regulation of Nephrin and Podocin expression to maintain the structure of protein complexes and the function of the integrity of slit diaphragm.
