乙肝病毒前C基因突变株感染者细胞免疫功能的观察

摘　　要：检测乙型肝炎病毒（ＨＢＶ）前Ｃ基因突变株感染者的细胞免疫水平，探讨细胞免疫与前Ｃ基因突变的关系。方法用多聚酶链反应（ＰＣＲ）结合地高辛标记的寡核苷酸探针杂交技术从６０名慢性乙肝患者中筛选出ＨＢＶ前Ｃ基因突变株感染者、ＨＢＶ野毒株感染者及ＨＢＶ已清除者各５人，用血源性ＨＢｓＡｇ和ＣＤ４＋Ｔ辅助细胞识别的抗原表位ＨＢｃＡｇ５０－６９合成多肽进行特异性淋巴细胞转化实验。结果ＨＢＶ前Ｃ基因突变株感染者对ＨＢｓＡｇ的应答与其余各组间差异不显著（Ｐ＞０．０５）；对ＨＢｃＡｇ５０－６９合成多肽的应答，与其余各组间差异有高度显著性（Ｐ＜０．０１）。结论ＨＢＶ前Ｃ基因突变株感染者对合成多肽的应答较野毒株感染者及ＨＢＶ已清除者低。Objective To study the cellular immunity of chronic hepatitis B patients with precore( pre c) mutation and the relationship between cellular immunity and pre C mutation.Method PCR combining digoxin tagged oligonucleotide probe was used to detect the pre C region of HBV DNA.5 cases patients with wild type HBV infection,5 with pre C mutants infection,5 with anti HBe positive,HBV DNA negative were selected.Result It was found that the CD 4+ T cell response to HBsAg was not significantly different between patients with pre C mutants infection and other groups(P>0.05),However,the CD 4+ T cell response to HBcAg50 69 synthetic peptide in the patients with pre C mutants infection decreased significantly comparing with the patients with wild type HBV infection and patients with anti HBe positive,HBV DNA negative(P<0.01).Conclusion The cellular immunity of chronic hepatitis B patients may relate to HBV pre C mutation.
