摘  要:目的:探讨舒芬太尼预处理对冷水束缚应激(WIRS)大鼠血浆促肾上腺皮质激素释放因子(CRF)的影响及其与急性胃黏膜病变(AGML)的关系。方法:成年雄性Wistar大鼠40只,随机分为4组(每组10只),包括正常对照组(NC组)、冷水束缚应激对照组(WC组)、舒芬太尼预处理组(SF组)和CRF阻断剂组(CB组)。AGML模型采用经典冷水束缚应激方法,水温(20±1)℃,6h后在麻醉状态下采集腹主动脉血以测定血浆CRF,并切除全胃以测定胃液pH值和观察胃溃疡指数(UI)。结果:WIRS可引起Wistar大鼠血浆CRF水平和UI显著升高(3.15±1.28vs.12.19±6.70;54.0±5.4vs.0.0±0.0,均P<0.01),胃液pH显著下降(1.58±0.16vs.2.63±0.21,P<0.01),而舒芬太尼和CRF阻断剂预处理可使WIRS大鼠血浆CRF水平和UI明显下降(6.93±4.06or3.3±1.4vs.12.2±6.7;18.8±2.6or13.3±1.9vs.54.0±5.4,均P<0.01)、胃液pH明显升高(1.95±0.08or2.47±0.23vs.1.58±0.16,均P<0.01)。结论:CRF可能参与了WIRS后AGML的病理过程;舒芬太尼预处理可有效地阻抑WIRS后AGML损害反应,可能与有效阻抑CRF的过度释放有关。Objective To investigate the effects of sulfentanyl pretreatment on plasma corticotrophin releasing factor (CRF) and its relationship with acute gastric mucosal lesion (AGML) induced by cold water immersion and restraint stress (WIRS). Medthods Forty healthy male Wistar rats were randomly divided into 4 groups (10 rats per group), including the normal control group(Group NC), WIRS control group(Group WC), sulfentanyl pretreatment group (Group SF) and CRF blocking agent group (Group CB). The AGML model were induced by classic WIRS for 6 hours in water with (20 ± 1)℃ temperature. Plasma CRFA was tested in bdominal aorta blood collected under anaesthesia, Gastric fluid pH value and the gastric ulcer index (UI) were observed after total gastrectomy. Results The plasma CRF and gastric UI of Wistar rats significantly increased (3.15 ± 1.28 vs. 12.19 ± 6.70; 54.0 ± 5.4 vs. 0.0 ± 0.0, P 0.01), and the gastric fluid pH significantly decreased (1.58 ± 0.16 vs. 2.63 ± 0.21, P 0.01),induced by WIRS. Under the pretreatment of both sufentanyl and CRF blocking agent, the plasma CRF and gastric UI significantly decreased (6.93 ± 4.06 or 3.3 ± 1.4 vs. 12.2 ± 6.7; 18.8 ± 2.6 or 13.3 ± 1.9 vs. 54.0 ± 5.4, P 0.01), while gastric fluid pH remarkably increased (1.95 ± 0.08 or 2.47 ± 0.23 vs. 1.58±0.16, P 0.01). Conclusions CRF may participate in the pathophysiological process of AGML induced by WIRS. Sufentanyl pretreatment can effectively restrain damage response of AGML induced by WIRS, which may be associated with effectively depress of CRF over-release.
