急性脑梗死患者血清基质金属蛋白酶-9测定的临床意义

摘　　要：目的探讨血清基质金属蛋白酶-9(MMP-9)含量对急性血栓形成性脑梗死(ACI)患者并发出血性脑梗死(HI)、多脏器功能障碍(MODS)影响的临床意义。方法应用酶联免疫吸附试验双抗体夹心法(ELSA),对72例ACI患者及50例正常人血清MMP-9定量分析,对72例血清MMP-9含量不同程度增高ACI患者并发HI、MODS发生率进行比较分析。结果 ACI患者血清MMP-9含量病后6 h明显高于正常人(P<0.01),病后2 d达最高,6 d恢复正常。ACI患者急性期血清MMP-9含量越高,HI、MODS发生率越高(P<0.01)。结论 ACI患者急性期血清MMP-9含量增高是CI发生的重要的病理学基础,MMP-9含量与HI、MODS发生率呈正相关,提示检测MMP-9含量可为临床评估ACI的病情及预后提供有价值的依据。Objective To explore the clinical significance of the influence of the content of serum matrixmetalloproteinase-9（MMP-9） on the patients suffering from acute cerebral infarction（ACI） associated with hemorrhagic CI（HI） and multiple organ dysfunction syndrome（MODS）.Mothods We analysed the content of serum MMP-9 in 72 ACI patients and 50 normal individuals respectively and compared the incidence of ACI associated with HI and MODS developing in 72 patients who had a diffent increase of serum MMP-9.Result The content of serum MMP-9 in ACI patients in 6 hours after the disease development was obviously greater than that in normal individuals（P0.001） which reached the peak in 2 days and returned to the normal level in 6 days.The greater the content serum MMP-9 in ACI patients at the ultra-early and acute phases,the more incidence of the disease would be.Conclusion The increase of serum MMP-9 was the pathological factors causing CI.The content of serum MMP-9 was proportional to the incidence of HI and MODS.It is indicated that the content of MMP-9 is quite valuable for the clinical evaluation of ACI.
