TRUS引导下6+X点穿刺活检法诊断前列腺癌的临床价值

摘　　要：目的:探讨经直肠超声(transrectal ultrasound,TRUS)引导下的6+X点穿刺活检方法对前列腺癌的临床诊断价值。方法:对88例疑似病例分别行TRUS引导下6点系统活检,及可疑X点定目标活检。结果:88例病人经穿刺取材病理证实,前列腺癌41例,良性前列腺增生41例,前列腺炎1例,前列腺增生合并炎症3例,非典型增生1例,前列腺结核1例。采用6+X点穿刺活检法对前列腺癌的检出率为46.6%,其灵敏度为95.3%,特异度及阳性预告值为100%。结论:TRUS引导下6+X点穿刺活检法可以提高前列腺癌的检出率,可为前列腺疾病的临床诊断,鉴别诊断及治疗方案的选择提供组织学依据。Objective：To explore the clinical value of prostate cancer with transrectal ultrasound （TRUS） and 6 ＋ X - point prostate biopsy under the TRUS - guided. Methods：88 cases were per- formed TRUS - guided biopsy systems 6 - point, subsequendy the suspected focuses - called X point was carried out additional biopsy. The positive ratio was compared between 6 point prostate biopsy and 6 ＋ X point prostate biopsy. Results： All cases were pathologically confirmed, including 41 cases of prostate cancer,41 cases of benign prostatic hyperplasia, 1 case of prostatitis,3 cases of benign prostatic hyperplasia combined with prostatitis, 1 cases of atypical hyperplasia, and 1 cases of prostate tuberculosis. The detection rate of prostate cancer through 6 ＋ X point biopsy system was 46.6~/0 , sensitivity ratio was 95.3% ,specificity and the positive predictive ratio were both 100%. Conclusion： 6 ＋ X - point biopsy method under TRUS - guided can improve the detection rate. It has a significance in the diagnosis and treatments in prostate cancer.
