指掌侧静脉吻合在手指中末节离断伤再植中的应用

摘　　要：目的在断指再植中采用吻合指掌侧静脉作为回流静脉，以增加手指再植的手术适应证及成活率。方法按常规再植的顺序，完成骨骼、肌腱、指固有动脉、指固有神经的修复后，再寻找并吻合指掌侧静脉。结果25例除1例因血管危象失败外，其余全部成活。再植后的功能评价：优者16例，良者5例，可者2例，差者1例。结论指掌侧静脉具有管径粗、位置恒定、抗损伤能力强、吻合方便的优点，能有效地缩短手术时间，极大地增加了断指再植的手术适应证及成活率，具有良好的临床应用价值。Objective To adopt the anastomosed venae digitals palmares in the replantation of severed finger as venous return to improve the operative indication and survival rate of finger replantatlon. Methods After finger bone,muscle tendon,proper arteria and proper nervus were repaired,following the routine orders of replantation,the venae digitales palmares was found and anastomosed. Results 25 cases were successful after replantation except one case of failure due to vascular crisis. The functions of fingers in 16 cases were evaluated as excellent,in 5 cases as good,in 2 cases as not bad and in one case as poor. Conclusion Venae digitales palmares have the following advantages,such as thicker calibre,constant position,stronger anti injury ability and easy to be anastomosed. So venae digitales palmares can effectively shorten the operative time,thus greatly increasing the operative indications and survival rate of replantation,and therefore be of excellent clinical value.
