北京自愿戒毒人群HIV感染率及其相关因素

摘　　要：目的了解北京自愿戒毒人群艾滋病病毒(HIV)感染情况及其相关因素,为在该人群中开展监测和行为干预提供依据。方法对2007年9-12月新入自愿戒毒所的戒毒者进行普查。结果共调查自愿戒毒所的研究对象833人,828人采集了血液标本,采血率99.4%。自愿戒毒所的戒毒者中,外地户籍占97.2%,其中99.0%的人在京逗留时间<1周。首次吸毒的中位年龄为28.42岁,首次注射吸毒的中位年龄为29.21岁,以海洛因为主。最近半年22.9%的人曾经注射毒品,4.9%曾经与别人共用针头/注射器。自愿戒毒所吸毒者HIV感染率为1.0%(8/828),自愿戒毒所戒毒者HIV感染的相关因素有共用针头/注射器(OR=7.572,95%CI=1.222～46.925)和维族(OR=87.582,95%CI=13.076～586.632)。结论自愿戒毒人群HIV检测无法反映北京市吸毒人群的状况,可考虑每年一次的自愿戒毒人群检测调整为间隔几年进行一次的专题调查,自愿戒毒所的干预重点是健康教育和HIV检测咨询。Objective To learn about the rate of HIV infection and its associated factors among durg users in voluntary detoxification centers（VDCs）in Beijing,so as to provide information for HIV surveillance and behavioral intervention in this particular population group.Methods Drug users recruited from VDCs in Beijing were surveyd from September to December of 2007.Results A total of 833 durg users from VDCs were interviewed,and blood samples were taken from 99.4% of them.Of the surveyed drug users 97.2% were residents from other regions,of whom 99.0% stayed in Beijing less than 1 week.The median age of starting drug use was 28.42 years and that of starting injection drug use was 29.21 years with heroine used as the main drug.Of them 22.9% reported injecting drugs in the last six months and 4.9% shared injection equipment with others.The rate of HIV infection among durg users from VDCs in Beijing was 1.0%（8/828）,The factors associated with HIV infection among them included sharing needles or syringes（OR=7.572,95%CI=1.222-46.925）and Ugur ethnic group（OR=87.582,95%CI=13.076-586.632）.Conclusion The HIV detection among drug users from VDC can not reflect the overall HIV prevalence among drug users in Beijing.It is suggested that specific survey be conducted among drug users in VDCs once in several years rather than HIV detection every year.Intervention among drug users in VDCs should focus on health education and HIV test and counseling.
