非绝经期女性冠心病患者血脂分析研究

摘　　要：目的研究分析非绝经期女性冠心病患者的血脂特点。方法选择2007年6月至2008年6月以冠心病或可疑冠心病住院的非绝经期女性患者157例。根据冠状动脉造影结果将其分为对照组（A组）和冠心病组（B组）。B组患者再根据病变血管Gensini评分法分为B1组、B2组和B3组。回顾分析A、B两组血脂变化特点。结果冠心病组甘油三酯（TG）、低密度脂蛋白胆固醇（IDL-c）、载脂蛋白（apo）B、脂蛋白Lp（a）水平明显高于对照组（P〈0．05），B1、B2、B3组血脂异常主要表现为TG、LDL-c和Lp（a）增高[（1．16±0．40）mmol／L、（1．85±0．93）mmol／L和（1．94±0．98）mmol／L，（2．43±0．60）mmol／L、（3．27±1．13）mmol／L和（3．70±1．13）mmol／L，（201±154）mg／L、（268±129）mg／L和（277±136）mg／L，P〈0．05]。结论非绝经期女性冠心病患者TG、LDL-C、apoB、Lp（a）水平明显升高，随着冠脉病变的加重，TG、LDL-C、Lp（a）明显升高，推测其为非绝经期女性冠心病患者的危险因素。Objective To study the blood lipid features of coronary heart disease in premenopausal women. Methods 157 premenopausal women with coronary heart disease or suspicious coronary heart disease were analyzed. Patients were divided into control group （group A） in which the coronary artery was normal, and CHD group （group B） by the results of coronary angiography. Group B were divided into group B1 ,B2 and B3 according to the score of coronary lesion used Gensini method. Results The level of triglyceride （TG）, lowdensity lipoprotein （LDL-C）, （apo）B, lipoprotein Lp （a） were significantly elevated in patients with coronary artery disease compare with control subjects （P〈0.05）. The level of TG,LDL-C and Lp （a） were significantly elevated in B1, B2, B3 [TG was （ 1.16±0.40）mmol/L, （ 1.85±0.93）mmol/L, （1.94±0.98）mmol/L respectively. LDL-C was （2.43 ±0.60）mmol/L, （3.27±1.13）mmol/L and （3.70±1.13）mmol/L. Lp （a） was （201 ±154）rag/L, （268±129）mg/L and （277±136）mg/L,P〈0.05）]. Conclusion The level of TG, LDL-C, （apo）B, Lp（a） are significantly elevated in patients with coronary artery disease. With the severity of the coronary artery lesion, the level of TG, LDL-C and Lp （a） are significantly elevated. We conclude that TG, LDL-C and Lp （a） are risk factors in premenopausal women with coronary heart disease.
