艾灸预处理对急性胃黏膜损伤大鼠血清中eHSP70及胃黏膜细胞NF-κB的影响

摘　　要：目的:探讨艾灸预处理对抗胃黏膜急性炎性损伤的机制。方法:健康SPF级SD大鼠32只随机分为正常对照组、模型组、艾灸组、艾灸非穴位组,每组8只,采用无水乙醇灌胃制备急性胃黏膜损伤模型。预先艾灸足三里、中脘等穴,以GUTH法检测胃黏膜损伤指数(UI)、HE染色光镜下观察胃组织形态学变化并进行炎症病理学评分;免疫组织化学法检测胃黏膜细胞中核因子(NF-κBp65)的表达、酶联免疫或放射免疫法测定血清中热休克蛋白(eHSP70)和炎性细胞因子TNF-α、IL-6、IL-8的含量。结果:与对照组比较模型组胃黏膜损伤指数、血清中eHSP70和炎性细胞因子含量明显升高(P<0.01)、胃黏膜组织炎性浸润明显,胃粘膜细胞NF-κBp65阳性表达明显升高(P<0.01)。与模型组比较,艾灸组胃黏膜损伤指数、血清中炎性细胞因子含量明显降低(P<0.05或P<0.01),胃黏膜组织形态较为完好,eHSP70含量升高(P<0.05),胃黏膜细胞NF-κBp65阳性表达下调(P<0.01)。2处理组比较,艾灸穴位组对上述指标的影响强于非穴组。结论:艾灸预处理对大鼠急性胃黏膜损伤具有一定的保护作用,其机制可能为诱导eHSP70升高,上调NF-κBp65的表达,减轻炎性细胞因子在胃黏膜的浸润,缓解急性胃黏膜损伤。艾灸穴位对胃黏膜的保护作用优于艾灸非穴位。Objective：To investigate the mechanism of moxibustion preconditioning against acute gastric mucosal injury.Methods：32 Sprague Dawley rats were randomly divided into 4 groups：control group（group A）,model group（group B）,moxibustion on points group（group C）,and moxibustion on no-points group（group D）,8 rats in each group.Acute gastric mucosal injury model was established by anhydrous ethanol gavage.Ulcer index（UI）was caculated by GUTH method,gastric mucosal histomorphology changes were observed after HE staining under light microscope,the expression of NF-κB p65 gastric mucosal cells was detected by immunohistochemistry,and the content of extracellular heat shock protein70（eHSP70）and inflammatory cytokines（TNF-α,IL-6,IL-8）in serum was measured by ELISA or radioimmunoassay.Results：Compared with control group,UI of model group and content of eHSP70,inflammatory cytokines in serum were significantly higher（P0.01）,while there was obvious inflammatory infiltration in gastric mucosa,the positive expression of NF-κB p65 increased slightly （P0.01）.And compared with model group,UI of moxibustion group,content of proinflammatory cytokines in serum were significantly lower（P0.01 or P0.05）,the tissue morphology was good.And the content of eHSP70 increased significantly （P0.01）.The expression of NF-κB p65 was down regulation（P0.01）.On above indexes the effect of moxibustion acupoints group was stonger than that of no-points group.Conclusion：Moxibustion preconditioning at Zusanli and Zhongwan acupoint had protective effect on acute gastric mucosal injury in rats,the mechanism may be related to raising the content of eHSP70,upregulating expression or NF-κB to inhibit the release of inflammatory cytokines,and relieve acute gastric mucosal injury.The effect of acupoints moxibustion was better than no-acupoints moxibustion.
