摘　　要：目的:探讨数字乳腺影像报告和数据系统(BI-RADS)5类与1类的乳腺密度等参数的差异及其与乳腺癌发生的相关性。方法:回顾性分析69例BI-RADS5类和137例BI-RADS1类的乳腺密度及年龄分布差异。经工作站分别测量BI-RADS5类乳腺及BI-RADS1类双侧乳腺平面径线,并记录相应乳腺厚度,对上述物理参数进行统计学分析。结果:与乳腺检查阴性组比较,高度怀疑乳腺癌的患者CC位平面径线较长,但两组乳腺厚度无显著差异(P=0.053)。乳腺密度在BI-RADS5类与1类两组间的差异有统计学意义(P=0.009)。二分类变量逻辑回归结果显示:年龄每增加10岁,乳腺癌检出率是前一年龄段的2.850倍(Waldχ2=28.723,P<0.001);乳腺密度每提高一个等级,乳腺癌检出率是前一密度等级的2.434倍(Waldχ2=13.671,P<0.001)。结论:年龄、乳腺密度是乳腺癌发生的重要危险因素,围绝经期及绝经后期中乳腺密度较高的女性是乳腺癌普查的重要对象。Purpose： To find the difference of breast density and other parameters between Breast Imaging Reporting and Data System（BI - RADS） category 5 and category 1, and to explore the correlation between breast parameters and incidence of breast cancer. Methods： Retrospective analysis was conducted on the distribution of breast physical parameters and ages of 206 cases, in which 69 BI - RADS patients and 137 BI - RADS 1 patients were contained. In the workstations, the plane diameters of bilateral breasts of each female patient were measured, and the corresponding breast compressed thickness were recorded. Data above were statistically processed. Results： By comparison, the highly - suspected breast cancer cases had much greater diameters in CC view, while breast compressed thickness in two groups had no significant difference （P = 0. 053）. Patients of BI - RADS category 5 had much higher breast density than category 1 （P = 0. 009）. Binary logistic regression analysis showed that the detection rate of breast cancer was 2. 850 times in every 10 years group from young to old in succession （Wald x2 = 28.723, P 〈 0. 001） and 2. 434 times in the higher density group compared with the successive lower group （Wald X2 = 13. 671, P 〈 0. 001）. Conclusion： Age and mammographic breast density are important risk factors of breast cancer, women with higher breast density during perimenopansal and postmenopausal periods are important targets in breast cancer screening.
