小切口椎板间开窗手术治疗腰椎间盘突出症的临床观察

摘　　要：目的评价小切口椎板间开窗术对治疗腰椎间盘突出症的临床疗效。方法随机选择2006年1月～2008年12月间采用小切口椎板间开窗术治疗的腰椎间盘突出症的患者60例,及半椎板切除术的患者24例,随访评价两组术后疗效。结果两术式术后疗效无显著性差异,但两术式在手术时间、术中出血及切口长度及术后卧床时间、术后住院时间、手术后恢复工作情况上都有显著性差异,前者明显优于后者。结论小切口椎板间开窗手术治疗腰椎间盘突出症术后疗效肯定,对脊柱稳定性破坏小,并发症少,是一种值得推广的治疗腰椎间盘突出症安全有效的手术方式。Objective To evaluate the curative effect of minimal invasive open discectomy in the treatment of lumbar disk herniation by comparing with the curative effect of semi-laminectomy.Methods Sixty patients undergoing minimal invasive opening disceetomy opera- tion and twenty four patients undergoing semi-laminectomy disceetomy from June of 2006 to December of 2008 were chosen by the random sampling method. All cases were reviewed in clinic by the table of ＂The graded method assess standard of curative effect＂, and the results were analyzed.Results There was no significant difference between the curative effects of the two methods. But there was a significant difference between two methods in operating time, blood loss, length of incision, the time lying in bed, length of stay and the time returning to work after operation. The minimal invasive opening diseectomy was significantly better than semi-larninectomy discectomy.Conelusion The minimal invasive opening discectomy is a safe and effective treatment of lumbar disc herniation. The curative effect is good and the destruction of the spinal column stabilization is very limited.
