摘 要:目的研究碳酸酐酶IX(carbonic anhydrase IX,CAIX)在鼻息肉组织中表达及其与嗜酸细胞(eosinophils,Eos)浸润的相关性,探讨CAIX在鼻息肉发病中的作用。方法收集28例鼻息肉组织(鼻息肉组)和12例正常下鼻甲组织(对照组),HE染色观察Eos浸润情况,免疫组化检测CAIX的表达。结果与对照组比较,鼻息肉组Eos浸润显著增加,CAIX免疫阳性细胞数及着色强度明显增高。图像分析显示,鼻息肉组CAIX的积分光密度(103/HP)为26.87±4.60,与对照组3.25±1.20相比,有显著性差异(P<0.05);Spearman等级相关分析显示,鼻息肉中CAIX表达与Eos浸润表达密切相关(r=0.632,P<0.01)。结论鼻息肉组织中CAIX表达、Eos浸润均上调,二者有协同表达关系,说明鼻息肉组织中存在缺氧及缺氧适应现象,CAIX表达上调,从而使细胞适应缺氧微环境,保证缺氧区域细胞的持续生长和增殖,促进Eos浸润生长,导致一种慢性炎症反应。OBJECTIVE To study the correlationof CAIX expression and eosinophils infiltration in nasal polyps, and to explore the role of CAIX in the pathogenesis of nasal polyps. METHODS Twenty-eight samples of nasal polyps and 12 samples of normal inferior turbinate mucosa were collected in this study. HE staining for eosinophils and immunohistochemistry method for CAIX was performed in two groups. RESULTS The numbers of eosinophils were higher in the nasal polyps than that in the control group, and the numbers and intensity of positive stained cells of CAIX were higher in nasal polyps. IOD of CAIX(103/HP)in the nasal polyps was 26.87 ±4.60 compared with 3.25±1.20 in the control group by image analysis. There were significant differences between two groups(P0.05). Spearman correlation test showed that there was a close correlation between CAIX expression and eosinophils infiltration in nasal polyps (r=0.632, P0.01). CONCLUSION The results indicate that CAIX and eosinophils are over expressed in nasal polyps, and there is a close correlation between the CAIX expression and the eosinophils infiltration. That shows there are hypoxia and hypoxia adaptation in nasal polyps and CAIX expression is up-regulated, so as to sustain normal pH of cells to adapt to hypoxic microenvironment and sustain persistent growth and hyperblastosis of cells in the hypoxic areas, and the infiltration and growth of inflammatory cells including eosinophils are promoted which leads to the chronic inflammatory reaction.
