CKD 3期患者随机尿蛋白/尿肌酐比值与24h尿蛋白定量的相关性分析

摘　　要：目的:分析随机尿蛋白/尿肌酐比值与24 h尿蛋白定量的相关性,探讨随机尿蛋白/尿肌酐比值是否可以替代24 h尿蛋白定量,检测尿蛋白的排泄情况。方法:对184例慢性肾脏病(CKD)3期患者同时留取随机尿和24 h尿液标本,分别进行随机尿蛋白,尿肌酐与24 h尿蛋白定量测定,计算随机尿蛋白/尿肌酐比值,与24 h尿蛋白定量结果进行相关性分析。应用ROC曲线分析24 h尿蛋白定量≥1.0 g和≥0.15 g时与随机尿蛋白/尿肌酐比值间的相对应关系。结果:随机尿蛋白/尿肌酐比值与24 h尿蛋白定量呈良好正相关(r=0.771,P<0.001)。应用ROC曲线分析随机尿蛋白/尿肌酐比值相应24 h尿蛋白定量1.00 g及0.15 g的诊断界点分别为0.83 g/gcr及0.16 g/gcr时敏感性与特异性最佳。结论:随机尿蛋白/尿肌酐比值可以反映CKD3期患者的尿蛋白排泄量,可替代24 h尿蛋白定量。Objective：To study the correlation between the ratio of random urine protein/urine creatinine and 24 h urine protein quantity,Discussing the ratio of random urine protein/urine creatinine,whether it can replace 24 h urine protein quantity.Monitor the excretion situation of urine protein.Methods：Specimens the random urine and 24 h urine from 184 cases of CKD 3 patients at the same time.Quantitative analysis the random urine protein,urine creatinine and 24 h urine protein respectively,to calculate the ratio of random urine protein/urine creatinine,then to make the correlation analysis with 24 h urine protein quantitative results.Application of ROC curve to analysis the relationship between 24 h urine protein（≥1.0 g,and≥0.15 g） and the ratio of the random urine protein/urine creatinine.Results：It showed a good positive correlation between the ratio of Random urine protein/urine creatinine and 24 h urinary protein.（r=0.771,P〈0.01）.Application of ROC curve to analysis the ratio of random urine protein/urine creatinine,there are diagnosis of community point at 0.83 g/gcr and 0.16 g/gcr when 24 h urinary protein are 1.00 g and 0.15 g respectively,the sensitivity and specificity is the best.Conclusion：The ratio of random urine protein/urine creatinine,it can replace 24 h urine protein.Monitor the excretion of urine protein of CKD 3 patients.
