摘 要:目的探讨单用动脉血PaCO2诊断吸氧后Ⅱ型呼吸衰竭的可行性。方法以吸氧前动脉血气分析结果为依据,373例老年病人分为Ⅱ型呼吸衰竭组和对照组,在吸氧后行动脉血气分析。分析氧合指数和PaCO2对吸氧后呼吸衰竭的诊断价值和两者之间相关性。结果氧合指数和PaCO2对Ⅱ型呼吸衰竭诊断的ROC曲线下面积各为0.94,0.93,与吸氧前动脉血气结果有较高一致性,(Kappa值0.76,0.84);PaCO2和氧合指数呈明显负相关(r=-0.94)。结论结合临床只用动脉血PaCO2诊断吸氧后Ⅱ型呼吸衰竭有一定可行性,值得进一步研究。Objective To investigate the feasibility of PaCO2 arterial blood alone for diagnosis of type Ⅱ respiratory failure after oxygen therapy.Methods Based on the results of arterial blood gas analysis before oxygen therapy,373 cases were divided into patients with type Ⅱ respiratory failure group and the control group,in the oxygen therapy later arterial blood gas analysis was done,the diagnostic value of oxygenation index and PaCO2 for respiratory failure after oxygen therapy and the relevance between the two were analyzed.Results The area under the ROC curve of oxygenation index and PaCO2 for the diagnosis of type Ⅱ respiratory failure was 0.94,0.93 respectively,had a higher consistency with results of arterial blood gases before oxygen therapy.(Kappa values was 0.76,0.84 respectively),PaCO2 was significantly negative correlated with oxygenation index(r=-0.94).Conclusion Diagnosis of Ⅱ respiratory failure after oxygen therapy by arterial blood oxygen PaCO2 combined with clinical has a certain feasibility,it is worthy of further study.
