彩色多普勒超声检测睾丸扭转的临床价值

摘　　要：目的探讨彩色多普勒对睾丸扭转的诊断价值。方法首先用二维超声显示１６例疑为睾丸扭转的睾丸附睾形态结构及内部回声，再用彩色多普勒观察血供状况，并全部经手术病理证实。结果１５例病侧睾丸血流消失或减少，睾丸内血流阻力指数增高，超声诊断睾丸扭转，与手术病理诊断相符；１例彩超未见血流减少，超声误诊为附睾炎。结论彩超诊断睾丸扭转正确率高（９３．８％），具有较高的敏感性和特异性，无创伤，简便易行，且能较准确地判断睾丸扭转的程度及病情发展的不同时期，为手术的选择提供了有效的依据，也可作为手术复位疗效评价的依据。Objective\ To investigate the value of color Doppler ultrasound (CDU) in the diagnosis of torsion of testis. Methods\ 16 patients with suspected torsion of testis were examined by 2D ultrasound first to display the morphological changes and the internal echoes of testis and epididymus, then the blood perfusion was observed by CDU. The results were compared with those of operation and pathology. Results 15 patients with disapeared or reduced testicular blood flow and increased resistence indexes diagnosed as torsion of testis were in accord with operative and pathologic results. 1 patient without reduced blood flow in testis was misdiagnosed as epididymitis. Conclusions\ The accurate rate of CDU to diagnose torsion of testis was 93.8%. It is a non traumatic highly sensitive and specific method in diagnosing torsion of testis, and can assess the degree and stages of torsion, help to select patient for operation and evaluate the efficacy of operation. It is the method of first choice to diagnose torsion of testis.
