摘 要:目的观察低氧诱导因子1α(HIF-1α)在高血压所致肾衰竭患者桡动脉中的表达。方法选择18例高血压所致肾衰竭患者为实验组,留取桡动脉标本。另选择8例腕外伤手术患者为对照组,留取与实验组相同位置处血管标本。对2组血管标本进行PAS染色,采用免疫组织化学法检测2组血管HIF-1α的表达。结果对照组桡动脉无明显中膜增厚及弹力纤维增生。与对照组比较,实验组桡动脉中膜增厚、平滑肌细胞增生、弹力纤维增多及出现内皮细胞损害。实验组桡动脉中膜厚度大于对照组[(0.45±0.16)mm比(0.31±0.12)mm](P〈0.05)。实验组HIF-1α表达于桡动脉中膜平滑肌细胞,内皮细胞也有少量表达;对照组HIF-1α仅少量表达于内皮细胞。实验组桡动脉HIF-1α表达较对照组高[(0.31±0.07)比(2.04±0.24)](P〈0.05)。桡动脉中膜厚度与HIF-1α表达呈正相关性(r=0.98,P〈0.05)。结论高血压所致肾衰竭患者桡动脉高表达HIF-1α,可能在高血压所致。肾衰竭患者血管改变中起作用。Objective To investigate the expression of hypoxia-inducible factor-1α (HIF-1α) in radial artery of patients with renal failure induced by hypertension. Methods Eighteen patients with renal failure induced by hypertension served as experimental group, and the radial vascular were collected. Eight traumatic patients served as control group, and normal vascular samples were taken out. The samples were stained by using PAS method, and the expression of HIF-1α in the vascular samples was detected hy using immunohistochemistry. Results As compared with control group, radial artery from experimental group had medial layer thickness, smooth muscle cell hyperplasia, higher expression of fibers and endothelial injury. Medial layer thickness of radial artery in experimental group was significantly thicker than that in control group [(0. 45 ± 0. 16) mm vs (0. 31 ± 0. 12) mm](P〈0. 05). In experimental group, HIF-1α was expressed mostly in medial layer and partly in endothelial cells, while it was expressed only in endothelial cells in control group. The expression of HIF-1α in experimental group was higher than that of control group [(0. 31 ± 0. 07) vs (2. 04± 0. 24)] (P〈0. 05) ,and its expression was positively related with medial thickness (r = 0. 98, P〈0. 05). Conclusions HIF-1α expression is much higher in radial artery, which may play an important role in the vascular changes of patients with renal failure induced by hypertension.
