硬膜外腔阻滞麻醉对脊髓生物学影响的实验研究

摘  要:目的探讨硬膜外腔阻滞麻醉对脊髓神经细胞的生物学影响。方法选用健康新西兰大白兔30只,随机分为实验组和对照组,每组15只。用2%戊巴比妥纳按30mg/kg,从兔耳缘静脉注射作基础麻醉。实验组用1%利多卡因作兔腰骶段硬膜外腔阻滞麻醉,对照组用生理盐水代替麻醉药。于麻醉后30min灌注与取材固定,蜡块包埋标本,切片处理,HE染色,免疫组织化学染色,图像采集分析及统计处理。结果 (1)硬膜外阻滞麻醉后,实验组相应节段脊髓灰质后角Ⅲ、Ⅳ板层的小圆细胞及前角Ⅸ板层外侧大多角细胞经HE染色后均有胞浆尼氏体减少,胞核偏向一端,其细胞形态改变与对照组比较差异有统计学意义(P<0.05);(2)免疫组织化学染色后,对照组上述细胞有c-fos蛋白表达,实验组上述细胞c-fos蛋白表达减弱或无表达,对照组与实验组表达量比较差异有统计学意义(P<0.05)。(3)腰骶段硬膜外腔阻滞麻醉后,脊髓软脊膜没有水肿或分层,与对照组比较差异无统计学意义(P>0.05)。结论硬膜外阻滞麻醉后,脊髓相应节段的神经细胞的功能受到抑制。硬膜外腔阻滞麻醉后,脊髓软脊膜没有水肿或分层现象。Objective To investigate the biological effects of epidural anesthesia through observation of the spinal cord nerve cells of the biological effects.Methods Thirty healthy New Zealand white rabbits were randomly divided into experimental and control group,15 in each group.With 2% satisfied by pentobarbital 30 mg/kg,from the edge of rabbit ears for the basis of intravenous anesthesia.Experimental group with 1% lidocaine for rabbit lumbosacral epidural anesthesia,the control group with normal saline in place of anesthetics.30 min after anesthesia and perfusion based on fixed,paraffin-embedded block samples,biopsy processing,HE staining,immunohistochemistry,image analysis were used to observe.Results(1)Epidural anesthesia,the experimental group the corresponding spinal dorsal horn gray matter Ⅲ,Ⅳ lamellar of small round cells,anterior horn of lateral lamellar Ⅸ most horn cells by HE staining Nissl bodies were reduced,one end of the nuclear bias,the cell morphological changes compared with the control group were statistically significant differences(P0.05);(2)Immunohistochemistry,the control group,these cells c-fos protein expression,compared with the experimental group were statistically significant differences(P0.05).(3)Lumbosacral epidural anesthesia,the spinal cord is not layered or soft spinal fracture compared with the control group no statistically significant difference(P0.05).Conclusion After epidural anesthesia,the corresponding segment of the posterior horn of the spinal cord gray matter Ⅲ,Ⅳ lamellar of small round cells,anterior horn of lateral lamellar Ⅸ most horn cells to reduce the Nissl bodies,tend to end nuclear,c-fos protein expression reduced or no expression.It sugguests that the corresponding segments of spinal cord nerve cells by inhibiting the function.After epidural anesthesia,spinal cord does not have a layered soft or breakage.
