乳腺浸润性导管癌的X线诊断价值

摘　　要：目的探讨乳腺浸润性导管癌(IDC)的X线诊断价值。方法回顾性分析经手术病理证实的212例IDC的乳腺X线表现。参考美国放射学会制定并推荐的BI-RADS及欧洲通用LeGals钙化形态分类标准。结果 212例乳腺浸润性导管癌(IDC)的主要X线征象:肿块165例(77.8%),微钙化80例(37.7%),肿块伴钙化60例(28.3%),单纯性钙化10例(12.5%),结构扭曲及非对称致密伴钙化10例(12.5%)。细小多形性钙化呈簇状分布40例,区域性分布20例,簇状伴线样20例。非对称致密18例(8.5%),结构扭曲13例(6.1%)。结论肿块、微钙化、肿块伴钙化、结构扭曲及非对称致密是IDC有价值的主要X线表现。分叶肿块、肿块边缘毛刺及细小多形性呈簇状伴线样钙化具有很高的诊断价值,束状毛刺更有价值。酒窝征、漏斗征、异常血管影、腋淋巴结肿大等次要征象对诊断也有一定价值。Objective To investigate the X-ray diagnosis value of breast infiltrating ductal carcinoma（IDC）.Methods The different types of mammographic appearance of 212 cases proved pathologically were retrospectively analyzed.Refer to the BI-RADS that was laid down and recommend by ACR and the LeGals classification of calcification′s morph standard used commonly in Europe.Results The main mammographic appearance of 212 patients with IDC include： mass（n=165,77.8%）,microcalcification（n=80,37.7%）,mass associated with calcification（n=60,28.3%）,simple calcification（n=10,12.5%）,architecture distortion and focal asymmetrical density with calcification（n=10,12.5%）.Extenuation polymorphy calcification in the form of clump and regional distribute were 40 cases and 20 cases respectively,in the form of clump with clew（n=20）,asymmetric density（n=18,8.5%）,architecture distortion（n=13,6.1%）.Conclusion The important and valuable image appearances of IDC were mass,microcalcification,mass associated with calcification,architecture distortion and focal asymmetrical density.There was vast diagnosis value of mass with lobulated,mass border′s barb and extenuation polymorphy calcification in the form of clump with clew,especially fasciculation barb.There were valuable secondary sign to the diagnosis,such as dimpling sign,funnel sign,abnormity angei,and axillary lymph nodes megaly etc.
