摘　　要：目的研究急性丙型病毒性肝炎患者外周血颗粒溶素(GNLY)的表达水平及其临床意义。方法收集30例急性丙型病毒性肝炎患者和30例体检健康者的全血和血清,分别行反转录聚合酶链反应(RT-PCR)测外周血单核淋巴细胞(PBMC)中GNLYmRNA的表达水平,酶联免疫吸附法(ELISA)法测血清中GNLY蛋白含量,并检测丙肝患者AST、ALT水平。结果急性丙型病毒性肝炎患者组PBMC中GNLYmRNA表达水平(7.71±1.44)高于体检健康者(0.83±0.17)(P<0.01);血清GNLY平均浓度[(20.16±6.03)ng/mL]也高于体检健康者[(9.94±2.99)ng/mL](P<0.01)。PBMC中GNLYmRNA表达水平与血清HCV RNA表达水平呈负相关(r=-0.869,P<0.01);血清GNLY含量与肝功能指标ALT呈正相关(r=0.522,P<0.01),与AST呈正相关(r=0.661,P<0.01)。结论急性丙型病毒性肝炎患者PBMC中GNLYmRNA表达和血清中GNLY蛋白含量显著高于健康人群,其表达水平的变化有助于了解患者机体细胞免疫状态,监测病情。Objective To investigate granulysin expression in peripheral blood of patients with acute viral hepatitis C and its clinical significance.Methods The level of GNLY mRNA in the PBMC and the content of serum GNLY were determinded by RT-PCR and ELISA respectively.Results The expression of PBMC GNLY mRNA in patients with acute hepatitis C（7.71±1.44） was higher than that of controls（0.83±0.17）（P0.01）;and serum GNLY average content [（20.16±6.03）ng/mL] was also higher than that of controls [（9.94±2.99）ng/mL]（P0.01）.The level of GNLY mRNA expression in PBMC had a negative correlation with the level of HCV-RNA expression（r=-0.869,P0.01）;the content of serum GNLY and the ALT was positively correlated（r=0.522,P0.01）,and was also positively correlated with AST（r=0.661,P0.01）.Conclusion The GNLY gene and protein expression levels are both higher in patients with acute viral hepatitis C compared with those in normal controls.The change level of GNLY expression may be helpful for determining of the cell immune status of acute viral hepatitis C patients,monitoring the condition of acute viral hepatitis C.
