摘  要:目的探讨rSj26-Sj32-IgM-ELISA法用于急性日本血吸虫病的诊断价值。方法用纯化的rSj26-Sj32融合蛋白和日本血吸虫成虫抗原(SjAWA)建立ELISA方法,检测急性日本血吸虫病患者血清中的特异性IgM抗体,并以华支睾吸虫病、卫氏并殖吸虫病、棘球蚴病、乙型肝炎、肺结核病患者和健康人血清作对照,比较二者的敏感性、特异性和交叉反应性。结果 rSj26-Sj32-IgM-ELISA和SjAWA-IgM-ELISA检测急性日本血吸虫病患者血清特异性IgM抗体的敏感性分别为98.0%和96.0%,特异性均为97.7%;SjAWA-IgM-ELISA检测华支睾吸虫病和卫氏并殖吸虫病患者血清时存在不同程度的交叉反应,而rSj26-Sj32-IgM-ELISA则未见交叉反应。结论纯化的rSj26-Sj32融合蛋白对急性日本血吸虫病具有较高诊断价值,可用作急性日本血吸虫病的诊断抗原。Objective To investigate the diagnostic value of using rSj26-Sj32-IgM-ELISA to diagnosis acute schistosomiasis japonicum.Methods Specific IgM antibodies were detected by ELISA in sera from patients with acute schistosomiasis japonicum prepared with purified rSj26-Sj32 protein and Schistosoma japonicum adult worm antigen(SjAWA).Sera were compared with sera from patients with paragonimiasis,clonorchiasis,echinococciasis,hepatitis B,and tuberculosis and from healthy individuals as control groups.The sensitivity,specificity,and cross-reactivity of rSj26-Sj32-IgM-ELISA and SjAWA-IgM-ELISA were compared.Results With sera from patients with acute schistosomiasis japonicum,rSj26-Sj32-IgM-ELISA and SjAWA-IgM-ELISA had sensitivity of IgM detection of 98% and 96%;both had a specificity of 2.33%.SjAWA-IgM-ELISA resulted in various degrees of cross-reactivity between paragonimiasis and clonorchiasis but rSj26-Sj32-IgM-ELISA did not.Conclusion Recombinant Sj26-Sj32 has diagnostic value and may be used as a diagnostic antigen for acute schistosomiasis japonicum.
