无创性血流动力学检测仪对房颤病人血流动力学的临床评价

摘　　要：目的应用无创性血流动力学监测仪评价持续房颤患者血流动力学的临床特点。方法以2006年3月—2010年1月住院的房颤病人57例为研究对象,同期住院非房颤病人57例为对照组。应用美国BioZ.Com无创血流动力学监测仪,选取18种血流动力学参数:心率(HR)、收缩压(SBP)、舒张压(DBP)、平均动脉压(MAP)、心输出量/心脏指数(CO/CI)、每搏输出量/每搏指数(SV/SI)、外周血管阻力/阻力指数(SVR/SVRI)、速度指数(VI)、加速指数(ACI)、液体水平(TFC)、左室功能/左室功能指数(LCW/LCWI)、预射血期(PEP)、左室射血分数(LVEF)、收缩时间比率(STR),对其血流动力学定量评估。结果房颤病人的CO/CI、SV/SI、VI、ACI较非房颤组低下(P<0.0 5);房颤组的PEP较非房颤组延长(P<0.0 5);房颤组LVET较非房颤组缩短(P<0.05);房颤组的STR较非房颤组增大(P<0.05)。结论房颤病人的左室功能低下。Objective To evaluate the hemodynamics of a trial fibrillation by the noninvasive hemodynamics monitor.Methods Fifty-seven inpatients with atrial fibrillation were selected and 57 inpatients with non atrial fibrillation were as controls.The hemodynamics of the patients with atrial fibrillation were evaluated by the American BioZ.Com Cardiodynamics Monitor with eighteen parameters,including heart rate（HR）,systolic blood pressure（SBP）,diastolic blood pressure（DBP）,mean arterial pressure（MAP）,cardiac output/index（CO/CI）,stroke volume/index（SV/SI）,systemic vascular resistance/index（SVR/SVRI）,velocity index（VI）,acceleration index（ACI）,left cardiac work/index（LCW/LCWI）,pre-ejection period（PEP）,left ventricular ejection time（LVET） and systolic time ratio（STR） in quantitation.Results The CO/CI,SV/SI,VI,and ACI of the patients with atrial fibrillation were lower than that of non atrial fibrillation patients（P〈0.05）.The PEP of atrial fibrillation patients was longer（P〈0.05）.The LVET of atrial fibrillation patients was shorted（P〈0.05）.The STR of atrial fibrillation patients was higher than that of non atrial fibrillation patients（P〈0.05）.Conclusion The left ventricular function were reduced in patients with atrial fibrillation.
