类风湿关节炎患者血清骨桥蛋白水平检测的临床评价

摘　　要：目的通过检测类风湿性关节炎(RA)患者血清中骨桥蛋白(OPN)的水平,探讨OPN水平与RA活动的相关性。方法收集临床确诊的258例RA患者的血清,其中活动期192例,非活动期66例,38例健康体检者血清,用酶联免疫吸附实验检测血清OPN水平,并与RA患者临床资料及实验室数据进行相关分析。结果 RA患者OPN水平明显高于健康体检者(P<0.01);活动期RA患者OPN水平明显高于非活动期(P<0.01);RA组OPN水平与C反应蛋白呈正相关(r=0.378,P<0.01),与类风湿因子呈正相关(r=0.878,P<0.01);与抗环瓜氨酸肽抗体呈正相关(r=0.632,P<0.01);与关节压痛数、红细胞沉降率呈正相关(P<0.05);与关节肿胀数无相关性(r=0.205,P>0.05)。结论 OPN参与RA发病,和风湿活动程度具有相关性,可作为评价RA活动度的指标之一。Objective To detect the serum osteopontin （OPN） levels in patients with rheumatoid arthritis and to investigate the relationship between OPN levels and rheumatoid arthritis （RA） activity. Methods 258 cases serums were collected with RA, among them 192 cases of active, 66 cases of non-active and 38 cases healthy volunteers. Enzyme-linked immunosorbant assay was used to detect the serum OPN levels. Clinical features of patients with RA and laboratory data were combined and their correlation was analyzed. Results The serum OPN levels in patients with RA were significantly higher than healthy volunteers （P 〈0.01）; and OPN levels in patients with active RA was significantly higher than in inactive RA （P 〈0.01）, and OPN levels in the RA group correlated positively with C-reactive protein （r =0.378, P 〈0.01）, and with rheumatoid factor （r =0.878, P 〈0.01）, and with anti-cyclic citrullinated peptide antibody （r =0.632, P 〈0.01）; and they also correlated positively with tender joint count, erythrocyte sedimentation rate （P 〈0.05）; but they had no correlation with swelling number of joint count （r =0.205, P 〉0.05）. Conclusion OPN involves in RA pathogenesis, and relates with the degree of rheumatoid activity. It can be used as an indicator of RA activity.
