脑创伤后炎症反应与循环内皮祖细胞的关系

摘  要:目的:探讨脑创伤后炎症反应与患者外周血中内皮祖细胞(EPC)动态变化的关系。方法:对71例脑创伤患者,根据其体温、血中白细胞数量等指标分为炎症组和非炎症组,采用流式细胞仪测定不同时间点的外周血EPC,并结合患者的其他临床特征进行比较分析。结果:所有患者创伤后24 h内外周血中的白细胞均显著升高,而EPC明显低于正常人(P<0.01),伤后4 d开始升高,7 d达到峰值,此后逐渐下降;伤后第4、7、14天炎症组患者白细胞数均显著高于非炎症组,但两组间血中EPC数量在第4、7天无显著差异,仅在伤后第14天,炎症组外周血中EPC明显高于非炎症组(P<0.001);非炎症组逐渐下降并回归正常。结论:炎症组相对于非炎症组脑创伤患者,炎症期外周血中EPC明显升高,可能为抗炎因子动员所至,为机体自身保护的启动。Objective: To investigate the dynamic relationship of endothelial progenitor cells(EPCs) and inflamematory in sub-acute phase of traumatic brain injury(TBI) in patients. Methods: 71 patients in subacute phase of traumatic brain injury were divided into inflammary and non-inflammary groups according to the temperature and white blood cells(WBC), the number of EPCs were examined by using flow cytometry, and the difference between inflammatory and non-inflammatory groups was analyzed. Results: The number of circulating EPCs of patients were significantly reduced within 24 hour after brain injury, elevated from 4 days later and got peak on 7 day, then reduced later. The mumber of EPCs in inflammatory patients were significantly higher than that in non-inflammatory patients in 14th day. The WBC in all patients were stress elevated, there were not difference between the two groups. But later, the WBC in non-inflammatory patients reduced to normal but was still high in inflammatory patients. And EPCs had correlation with WBC. Conclusion: EPCs are elevated later in inflammatory patients, the reasons maybe is that EPCs are mobilized to protect the body by anti-inflammatory factors.
