摘  要:目的:探讨腰椎间盘退变中终板骨软骨炎的低场强MRI表现及其临床意义。方法:经临床及影像学确诊的腰椎终板骨软骨炎患者68例,其中男性37例,女性31例;年龄32~79岁,平均52岁,分析终板及邻近骨质信号改变。结果:68例病例中共累及终板186个,MRI表现为沿终板及其邻近椎体内带状或斑片状异常信号灶,按Modic分型其中Ⅰ型57个,占30.6%,Ⅱ型109个,占58.6%,Ⅲ型20个,占10.8%。好发于下腰段。所有病例均伴有椎间盘变性或突出,骨质增生54例,Schmorl结节30例,脊柱不稳16例。结论:低场强MRI能清楚显示腰椎间盘退变中终板骨软骨炎各期的信号特点,对病灶显示清晰,能准确分型,有利于临床制定合理的治疗方案。Objective: To study the low-field MRI imaging and clinical significance of end-plate osteochondritis. Methods:80 patients with end-plate osteochondritis were collected. There were 37 males and 31 females with average of 52 years(32-79). The signal changes in end-plate and its adjacent vertebral bones were analyzed. Results: Of 68 cases,end-plate osteochondritis was identified 186. The strip or patchy abnormal signal along the end-plate and adjacent verterbral was showed on MRI. It had been classified into 3 types according to Modie classification. 57 ( 30.6% ) were Modic type Ⅰ , 109 ( 58.6% ) were Modic type Ⅱ and 20 ( 10.8 %) were Modic type Ⅲ. End-plate osteochondritis was often located at inferior segment of lumbar spine. All cases were accompanied by lumbar disc degeneration and other changes including hypcrosteogeny in 54 (79.4%), Schmorl's nodule in 30 (44% )and spinal instability in 9 (13.2 %). Conclusion: Abnormal signal intensities of different phases for end-plate osteoehondritis can be displayed clearly by low-field MRI,whieh is helpful in treatment planning for lumbar disc degeneration.
