摘  要:目的评价微量肉汤稀释法检测葡萄球菌诱导型克林霉素耐药的应用价值。方法分别应用微量肉汤稀释法和D试验法检测88株红霉素耐药、克林霉素中介或敏感葡萄球菌属(其中金黄色葡萄球菌36株、凝固酶阴性葡萄球菌52株)中的诱导型克林霉素耐药菌株,两种方法的结果进行配对χ2检验,同时以D试验法为"金标准"计算微量肉汤稀释法的敏感性、特异性和Kappa值。结果经配对2χ检验,两种方法的检测结果差异无统计学意义,微量肉汤稀释法检测金黄色葡萄球菌诱导型克林霉素耐药的敏感性、特异性均为100.0%,Kappa值为1.0;检测凝固酶阴性葡萄球菌诱导型克林霉素耐药的敏感性为100.0%、特异性为96.6%,Kappa值为0.962。结论微量肉汤稀释法检测葡萄球菌属诱导型克林霉素耐药与D试验法具有很好的一致性。OBJECTIVE To evaluate the clinical application for detection of inducible clindamycin resistance in staphylococci by broth microdilution.METHODS A total of 88 staphylococci isolates [including 36 Staphylococcus aureus isolates and 52 coagulase-negative Staphylococcus(CNS) isolates] which were erythromycin resistant but clindamycin susceptible or intermediate were collected and detected by broth microdilution and D-zone test.The results were analyzed statistically by χ2(chi-square test) and Kappa analysis to used to compare the difference between two detection methods.RESULTS The detection results of inducible clindamycin resistance in staphylococci by broth microdilution and D-zone test were without significant difference(P0.05).The sensitivity and specificity of broth microdilution method were 100% and 100% vs 100% and 96.6%,and the kappa value was 1.0 and 0.962 in S.aureus and CNS,respectively.CONCLUSION The detection results of inducible clindamycin resistance in staphylococci by broth microdilution show a excellent concordance with D-zone test.
