摘  要:目的调查多药耐药铜绿假单胞菌中接合性质粒遗传标记、插入序列遗传标记、转座子遗传标记、整合子遗传标记等可移动遗传元件的存在情况。方法收集复旦大学附属上海市第五人民医院2009年5-11月分离自住院患者送检痰液标本的铜绿假单胞菌共20株,采用聚合酶链反应(PCR)及序列分析的方法分析接合性质粒遗传标记(traA、traF、trbC)、插入序列遗传标记(ISCR1、ISCR3/14、IS1133、ISpa7、ISEcp1)、转座子遗传标记(tnpU、tnpA/Tn21、tnsA)、整合子遗传标记(intⅠ1、intⅠ2、intⅠ3)共14种可移动遗传元件。结果 20株铜绿假单胞菌共检出trbC、ISCR1、ISpa7、tnpA/Tn21、intⅠ1等5种基因,其阳性株数分别为10株(50.0%),12株(60.0%),12株(60.0%),12株(60.0%),11株(55.0%),其余9种基因未检出;阳性检出基因可分为3种构成模式,其中(trbC+ISCR1+ISpa7+tnpA/Tn21+intⅠ1)模式检出率最高,为9株(45.0%)。结论质粒、插入序列、转座子、整合子4类可移动遗传元件在本组铜绿假单胞菌中均有检出,这些可移动遗传元件介导各种耐药基因使受体菌表现为多药耐药;同时检测14种可移动遗传元件是国内首次报道,检出tnpA/Tn21、Ispa7为国内首次报道。OBJECTIVE To investigate the distribution of mobile genetic elements:plasmids,insertion sequences,transposons,and integrons in multidrug-resistant Pseudomonas aeruginosa(MDRPA) strains.METHODS From May 2009 to Nov 2009,20 strains of MDRPA isolated from sputum were collected from Shanghai No.5 People′s Hospital affiliated to Fudan University.Then,14 kinds of mobile genetic elements(traA,traF,trbC,SCR1,ISCR3/14,IS1133,ISpa7,ISEcp1,tnpU,tnpA/Tn21,tnsA,intⅠ1,intⅠ2,and intⅠ3) were analyzed by PCR and verified by DNA sequencing.RESULTS Among 20 strains of MDRPA,10 strains(50.0%),12 strains(60.0%),12 strains(60.0%),12 strains(60.0%),and 11 strains(55.0%)were detected to carry trbC,ISCR1,ISpa7,tnpA/Tn21,and intⅠ1,respectively.However,the rest 9 kinds of genes couldn′t be detected.Positive genes could be divided into 3 kinds of positive modes,and the mode(trbC+ISCR1+ISpa7+tnpA/Tn21+intⅠ1) had the highest rate(9 strains,45.0%).CONCLUSION Four kinds of mobile genetic elements including plasmids,insertion sequences,transposons,and integrons are all detected in this group of 20 strains of MDRPA,and acquired resistance genes induced by these mobile genetic elements cause the recipient strains to express multi-drug resistance.It′s the first report to detect 14 kinds of mobile genetic elements in China and it′s the first report that tnpA/Tn21 and Ispa7 are detected in China.
