心脏停跳期间气道持续正压对肺顺应性的影响

摘  要:目的探讨不同的气道持续正压对大鼠心脏停跳期间静态胸廓—肺顺应性(STLC)的影响。方法将28只Wistar大白鼠随机分为4组,P0组(n=7):不施加持续气道正压;P5组(n=7):施加5cmH2O的持续气道正压;P10组(n=7):施加10cmH2O的持续气道正压;P15组:施加15cmH2O的持续气道正压,气道内加压前及气道内加压后的30分钟、60分钟、90分钟及120分钟分别测量STLC。结果气道内加压前各组间的STLC无显著差异(P>005),但在加压30分钟、60分钟、90分钟及120分钟后均明显低于加压前的值(P<005),而以P5及P15组的下降最为明显。加压60分钟后,P10组的STLC明显高于P5、P15组(P<005)。加压后120分钟,不但P10组的STLC明显高于P15及P5组,P0组也明显高于P15及P5组(P<005)。在整个观察期间P10组与P0组间无明显差异,同时P5组与P15组间也无显著差异(P>005)。结论气道内加压10cmH2O可预防静态胸廓—肺顺应性的显著下降。Objective To study the effects of different continuous positive airway pressure (CPAP) on static thoracic wall-lung compliance (STLC) during cardiac arrest in rats. Method Twenty-eight Wistar rats were randomly divided into four groups,P0(n=7):without CPAP(no airway pressure was added);P5(n=7):with 5 cmH2O CPAP;P10(n=7):with 10cm H2O CPAP;P15(n=7):with 15cmH2O CPAP.The STLC was measured 30min60min90min120 min after giving CPAP. ResultsThere were no significant differences in STLC among four groups before giving CPAP(P>005).But the values of STLC reduced obviously after giving CPAP than that of no CPAP(P<005),Especially that in group P10 and group P5 and P15 at 60 min after giving CPAP(P<005).Not only the STLC of group P10 was more higher than that of group P15 and group P5,but also the STLC of group P0 at 120 min after giving CPAP.There was no singificant difference between group P10 and group P0,group P5 and group P15 in the period of whole investigation.HZConclusion Giving 10 cmH2O CPAP plays an effective role in preventing STLC from reducing obviously.
