腹部闭合性损伤患者血浆内皮素水平的观察

摘　　要：目的探讨血浆内皮素（ＥＴ）在不同程度的腹部闭合性损伤患者治疗前后含量变化规律及临床意义。方法按休克程度将３５例腹部闭合性损伤患者分为轻、中、重三组，采用放射免疫分析法测定３５例患者治疗前后及２１例健康者ＥＴ含量。结果治疗前患者ＥＴ含量明显高于对照组，且休克越重升高越显著（６６１±１０２ｎｇ／Ｌ、８１３±１２８ｎｇ／Ｌ、８９４±１３７ｎｇ／Ｌ比４８２±１０２ｎｇ／Ｌ，Ｐ＜００５、Ｐ＜００１、Ｐ＜０００５）；轻度患者治疗后ＥＴ水平降至正常（５０８±９９ｎｇ／Ｌ），中度者较治疗前明显下降（６４６±９３ｎｇ／Ｌ，Ｐ＜００５），但仍显著高于对照组（Ｐ＜００５），重度者治疗前后比较无显著变化（８８７±１５９ｎｇ／Ｌ，Ｐ＞００５）。结论血浆ＥＴ水平变化的测定，对于腹部闭合性损伤患者具有重要的临床参考价值。Objective To investigate the change of plasma endothelin (ET) levels in patiens with different degree of abdominal closed injury and its clinic singificance . Methods Abdominal closed injury patients were classified into three groups: minor,moderate and severe acoording to the degree of shock .The plasma ET levels were measured by RIA in 35 patients with abdominal closed injury before and after treatment and in 21 healthy volunteeres. Results The levels of plasma ET in the patients before treatment were higher than those in normal subjects . And the serious their shock degree were ,the higher the levels rose (661102 ng/L813128 ng/L894137 ng/L482102 ng/L,P<005P<001P<0005);After treatment the ET levels in the monir and moderate groups (64693 ng/L,P<005)decreased significantly ,but the levels remained high high in the moderate groups (P<005),while decreased to normal in the minor group (50899 ng/L,P>005) no significant difference were observed in the severe group before and after treatment (887159ng/L,P>005). Conclusion The measurement of plasma ET levels may be of great value in patients with abdominal closed injury.
