紫草对尖锐湿疣患者皮损中Bcl-2、survivin及VEGF表达的影响

摘　　要：目的探讨中药紫草治疗和防止尖锐湿疣（CA）复发的机制。方法 60例CA患者分为治疗组和对照组各30例,治疗组外用紫草煎剂治疗,治疗前及治疗2周后取同一部位的皮损,分别采用免疫组织化学二步法检测并比较皮损中Bcl-2、生存素（survivin）、血管内皮细胞生长因子（vascular endothelial growth factor,VEGF）的表达水平的变化。对照组予电灼去除疣体,伤口外涂百多邦软膏,根据创面大小适量服用少量抗生素。比较两组的治疗效果及随访结果。结果治疗组经紫草治疗后,CA皮损中Bcl-2、survivin、VEGF的表达水平均明显低于治疗前（P〈0.05）。治疗组的治愈率高于对照组（P〈0.01）,复发率低于对照组（P〈0.01）。结论紫草可能通过下调凋亡抑制因子Bcl-2、survivin的表达,促进感染细胞凋亡,同时下调VEGF的表达,抑制疣体供血血管的增生,起到清除病毒、使疣体缩小乃至消退的作用。Objective To explore the mechanism of decoction of gromwell root in treatment and prevention of the recurrence of genital warts. Methods 60 patients with genital warts were randomized into treatment group and control group,with 30 patients in each group. The patients in the treatment groups were treated with the decoction of gromwell root. The expression of Bcl-2,survivin and VEGF from skin lesions of 30 patients with genital warts were detected by immunohistochemistry,the lesions were taken from the same site of the patients before and after the treatment with decoction of gromwell root. The therapeutic effects and follow-up results of the patients in the two groups were also compared. Results After the treatment,the expressions of Bcl-2 and surviving,VEGF from skin lesions after treatment were significantly lower than those before the treatment with the decoction of gromwell root （P0.05）. The recovery rate of the treatment group was higher than the control group,while the recurrence rate in the treatment group is lower than that in the control group （P0.01）. Conclusion Decoction of gromwell root could down-regulate the expressions of Bcl-2,surviving,and VEGF in skin lesions of genital warts,promote apoptosis of.the infected cells,inhibit the angiogenesis of the warts,to achieve the effects of scavenging virus,shrinking warts and even cleaning up warts.
