摘 要:目的:探讨应用无框架立体定向技术结合显微手术微侵袭治疗脑内小病灶的方法及疗效。方法:应用CAS-R-2型无框架立体定向仪和CAS-BH5型机器人对33例脑内小病灶患者,行术前精确定位后,在显微镜下切除病灶,并观察疗效。结果:本组病变全切除30例,占90.9%,次全切除2例,占6.0%,大部分切除1例,占3.0%,无手术死亡。本组19例症状性癫痫起病的患者,15例(78.9%)病灶清除后症状消失,4例(21.0%)发作次数明显减少,效果良好。结论:采用无框架立体定向技术微侵袭治疗脑内小病灶,具有微创伤、精确性高、可重复性好及安全性强等优点,是临床疗效满意的治疗手段。Objective :To discuss the experience and value of Frameless Stereotaxic techniques to treatment intracranial small lesions.Methods:Intracranial small lesions of 33 patients were operated by standard in microsurgical,which were guided from the type of CAS-R-2 Frameless Stereotaxis techniques and the type of CAS-BH5 robot.Results :Total resection was performed in 30 patients(90.9%),subtotal resection was performed in 2 patients(6.0%),main resection was performed in 1 patient(3.0%).The patients with intracranial small lesions without complication and mortality.Of the 19 patients with secondary epilepsy,15(78.9%) were seizurefree and 4 were significantly decreased seizure frequency,the effect was significant.Conclusion:Using the frameless stereotaxic techniques to treatment intracranial small lesions,is a mini-invasive,accurate,safety and effective method for the micro-surgical.
