摘 要:目的探讨卵巢上皮性肿瘤黑色素瘤细胞黏附因子(melanoma cell adhesion molecule,M-CAM)的表达与微血管密度(microvessel density,MVD)和临床病理特征之间的关系。方法 2007年1月至2008年1月在重庆医科大学附属第一医院应用半定量逆转录聚合酶链反应(RT-PCR)、Western blot、免疫组化检测8例正常卵巢组织、16例卵巢良性上皮性肿瘤(以下简称卵巢良性肿瘤)、40例卵巢上皮性癌(以下简称卵巢癌)组织及20例大网膜转移灶中M-CAM的表达。用CD34标记免疫组化法检测MVD。结果 M-CAM mRNA在卵巢癌细胞可见明显扩增条带,M-CAM mRNA表达值在癌组织和大网膜转移灶中高于卵巢良性肿瘤和正常卵巢组织(P<0.05)。Westernblot显示M-CAM蛋白在正常卵巢组织、卵巢良性肿瘤、卵巢癌组织、大网膜转移灶中M-CAM蛋白的表达值分别为0.314±0.162、0.349±0.135、0.603±0.093、0.636±0.102。免疫组化显示卵巢癌组织的MVD为42.53±8.15,卵巢良性肿瘤的MVD值为20.42±6.07,两者差异有统计学意义(P<0.05)。卵巢癌组织中M-CAM阳性表达组的MVD高于M-CAM阴性表达组。M-CAM蛋白高表达与卵巢癌临床分期、淋巴结转移有关,而与年龄、病理类型、有无腹水、CA125水平等无关。结论 M-CAM的表达不仅与卵巢肿瘤的良、恶性有关,而且与卵巢癌的进展、转移有关;与卵巢癌血管形成有关。Objective To investigate the expression of M-CAM and CD34 in the human ovarian epithelial carcinoma and whether there is a correlation between M-CAM expression and clinicopathological features of ovarian epithelial carcinoma. Methods Between January 2007 and January 2008 in First Affiliated Hospital of Chongqing Medical University. Eight specimens of normal ovarian tissue,16 cases of ovarian epithelial benign tumor,40 cases of ovarian epithelial carcinoma and 20 cases of metastatic focus on omentum were examined. Semi-quantitative reverse transcriptionase polymerase chain reaction(RT-PCR) and Western blot analysis were used to detect mRNA and protein expression of M-CAM respectively. MVD in tumor was assessed immunohistochemically. Results M-CAM mRNA was detected in ovarian carcinoma tissue. M-CAM mRNA expression was higher in ovarian epithelial carcinoma and metastatic focus on omentum than that in ovarian epithelial benign tumor and normal ovarian tissue ( P 〈 0. 05 ). Western blot showed M-CAM protein expression was 0. 314 ±0. 162,0. 349 ±0. 135,0. 603 ±0. 093 and 0. 636 ±0. 102 respectively in normal ovarian tissue, ovarian epithelial benign tumor, ovarian epithelial carcinoma and metastatic focus on omentum. MVD in the ovarian epithelial carcinoma tissue was significantly higher than in the ovarian epithelial benign tumor tissue (42. 53 ± 8. 15 vs 20. 42 ± 6. 07, P 〈 0. 05 ). MVD in positive expression of M-CAM in the ovarian epithelial carcinoma tissue was higher than that in the negative tissue. High expression of M-CAM mRNA and protein were associated with clinical stages and lymphnode metabasis but not with age, pathological types,ascites and degrees of CA125. Conclusion High M-CAM expression may benot only related to benign tumor of the ovary and malignant tumor,but also to progression and metastasis of ovarian carcinoma. M-CAM may play important roles in angiogcnesis of malignant tumor.
