胸腰段椎体爆裂骨折并完全性瘫痪行前路手术患者围手术期的护理

摘　　要：目的探讨胸腰段椎体爆裂骨折并完全性瘫痪行前路手术患者围手术期的护理经验。方法对22例胸腰段椎体爆裂骨折并完全性瘫痪患者均行前路手术,做好围手术期的护理,术后随访1-5年。结果 22例患者术前肋缘下至腹股沟束榨感及腰背痛全部消失,3例感觉平面下降8-18cm,而且分别在术后4个月、8个月、12个月恢复部分排尿功能。全组患者双下肢肌力无恢复,8例于3个月之后出现不同程度肌张力升高。结论做好心理护理是保证各种治疗和护理顺利进行的基础;病情观察和脊髓神经功能的观察,体位护理和功能康复训练是促进患者早日康复的关键。Objective To sum up the experience of nursing patients with bursting fractures at thoracic lumbar vertebrae combined with complete palsy managed with anterior approach.Methods 22 patients with bursting fractures at thoracic lumbar vertebrae combined with complete palsy were managed with anterior approach and nursed perioperationally.All were followed up from 1-5 years.Results The sense of pressing and pains along anterior costal margin to fold inguen disappeared for all the patients.In three of them,the feeling of plane was lowered by 8-18cm.All resumed their urination at months 4,8 or 12 after operation.For all,the animal force got not recovery in both lower extremities and muscular tension was increased at various degree 3 months after operation in 8 patients.Conclusion Mental care is fundamental for smooth treatment and care.Observations over pathogenetic conditions and spinal neurofunctions,body position nursing and functional rehabilitation training are critical for the quick rehabilitation of patients.
