摘  要:目的:应用多普勒超声评价二尖瓣位人工金属瓣的远期效果。方法:选择44例患者,男16例,女28例,年龄15~64岁(平均46岁)。换瓣时间12~96个月(平均48个月)。对比分析术前、术后近期(<12个月)和术后远期(>12个月)及正常自然瓣膜(20例)的二尖瓣口血流动力学参数。结果:术后远期人工瓣仍有较好的血流动力学,远期疗效与近期同样显著,人工瓣速度参数、压力参数和二尖瓣口有效面积均有明显改善,P<0.0001;同自然瓣膜比较人工瓣在术后近、远期仍有明显梗阻,P<0.0001;双叶瓣效果略好于单叶瓣,但两者无显著差异,P>0.05。左房内径近期有回缩,但远期仍可再次增大。Objective:To assess the long time effect of mechanical prostheses in mitral position.Methods:44 patients having their implantations for 12 96 months(mean 48 months)were examined by Doppler echocardiography.Doppler parameters of mitral hemodynamics were comparatively analysed in all patients.Results:Long time mitral prostheses had significantly beneficial hemodynamics for a short time.Vmax,Vmean,PGmax,PGmean,PHT,MVA of mitral prostheses improved significantly.Native valve prostheses had obstruction both at short and long time after implantation.Bileaflet valves were better than tilting disc valves,no significant difference between them.Left atrum diameter reduced for a short time,but increased again as times get on.
