摘 要:目的探讨一氧化碳(CO)等在小儿哮喘发病机制中的作用及意义。方法应用Chalmers血红蛋白结合及联二亚硫酸盐还原法检测了22名正常儿童、26例支气管哮喘患儿血浆CO水平,同时测定了一氧化氮(NO)和肿瘤坏死因子α(TNFα)水平,并在动物模型上进行验证。结果哮喘患儿急性发作期血浆CO、NO2/NO3和TNFα水平与对照组比较,t值分别为8421,3088,15432,(P均<0.01),缓解期CO和NO2/NO3水平恢复正常(P均>0.05),而TNFα水平与对照组比较差异有显著性(P<0.01)。哮喘患儿血浆CO与NO2/NO3水平呈显著正相关(r=0.476,P<0.05),而与TNFα相关不显著(r=0.268,P>0.05)。过敏性哮喘豚鼠血浆CO和TNFα水平与对照组比较,t值分别为3694,7454,(P均<0.01),而NO2/NO3水平变化差异无显著性(P>0.05)。过敏性哮喘豚鼠支气管肺泡灌洗液(BALF)中CO、NO2/NO3和TNFα水平的t值分别为3049,5609,5955,(P<0.05~001)。糖皮质激素组血浆及BALF中?Objective To investigate the role of carbon monoxide (CO) in the pathogenesis of asthma. Methods We examined CO, nitrite and nitrate (NO 2/NO 3) and tumor necrosis factor alpha (TNF α) levels in plasma of 26 asthmatic children and 22 matched normal children. At the same time, we examined the plasma and bronchoalveolar lavage fluid (BALF) CO, NO 2/NO 3 and TNF α levels in guinea pigs with allergic asthma. Results Asthmatic children in acute attack stage had increased levels of CO, NO 2/NO 3 and TNF α in plasma compared with normal control subjects (all P <0.01), while in stable stage plasma CO and NO 2/NO 3 reduced to normal levels (both of the P >0.05), but TNF α was still higher than the normal controls ( P <0.01). Guinea pigs with allergic asthma had increased amounts of CO and TNF α in plasma and CO, NO 2/NO 3 and TNF α in BALF as compared with normal control subjects ( P <0.05~0.01), but no changes in levels of circulating NO 2/NO 3 ( P >0.05) were found. There was no difference between glucocorticoid treated asthmatic guinea pigs and normal controls in levels of CO, NO - 2/NO - 3, and TNF α. The elevation of plasma CO correlated with elevated NO 2/NO 3 level in asthmatic children ( P <0.05), so did in the BALF in allergic guinea pigs. Conclusions Our present data showed that endogenously produced CO was increased during the course of asthma, which suggested that the role of CO in asthma might worth further study.
