异丙酚复合芬太尼兔颈总动脉麻醉的可行性研究

摘  要:目的探讨异丙酚复合芬太尼经颈内动脉麻醉的可行性。方法新西兰大白兔60只,随机分为2组,静脉芬太尼组V组(n=30),动脉芬太尼组A组(n=30),又将2组各分为2个亚组,Vv组(静脉芬太尼复合耳缘静脉异丙酚n=15),Va组(静脉芬太尼复合颈总动脉异丙酚n=15),Av组(动脉芬太尼复合耳缘静脉异丙酚n=15),Aa组(动脉芬太尼复合颈总动脉异丙酚n=15)。监测生命体征及脑电波。观察不同麻醉方法到达相同麻醉深度时呼吸、循环参数的变化,记录麻醉诱导时间与苏醒时间、用药量,并观察兔的饮食、行为有无异常及血管内皮细胞的病理变化。结果各亚组之间比较A组异丙酚用药量明显减少,而芬太尼用量无差异。异丙酚通过颈总动脉给药后意识消失、达到脑电静息的时间均短于V组。4组对于呼吸、循环均有不同程度的影响,其中Vv组的影响最大,Aa组的影响最小。术后60只大白兔全部存活,每组随机取5只观察48h,未发现在行为、进食水、性情等方面有异常表现。兔血管内皮光镜结构改变:Vv组血管内膜光滑连续,着色均匀,其余3组血管结构形态与Vv组比较无明显改变,未见任何急性损伤征象。结论经颈总动脉持续输注异丙酚及芬太尼可以安全有效的达到维持兔全身麻醉的目的;与静脉给药比较,具有麻醉诱导快、用药量少、血流动力学稳定、呼吸抑制轻等优点。Objective To investigate the feasibility of intracarotid propofol and fentanyl infusion for general anesthesia in rabbits. Methods The experiments were performed on 60 adult rabbits. These animals were divided into two groups randomly of each 30, including group V ( intravenous administration of fentanyl group, n = 30) and group A (the common carotid arterial administration of fentanyl, n = 30). Then each group was divided into two subgroups :group Vv( n = 15) ,group Va( n = 15) ,group Av ( n = 15) and group Aa( n = 15). The outcomes were recorded,including the time of consciousness loss and recovery of consciousness, the dose of propofol and fentanyl. The changes of hemodynamics and respiration in rabbits at the same depth of anesthesia were observed. Animals were executed after experiment and the common carotid artery tissue of infusing drugs were obtained and detected by light microscope . Results The dose of common carotid arterial propofol was significantly less than that of intravenous propofol at the same depth of anesthesia, while the dose of fentanyl has no significant difference between the two administration routes. The respiration and circulation were all influenced in the 4 groups, among which the influence on group Vv was the most,while that of group Aa was the least. As for loss of consciousness, carotid artery propofol were more quickly than intravenous infusion. The eudothelium of control group was smooth and glossy and the coloration was uniform. So does the experiment group. There was no any acute injury by light microscope. Conclusion The different methods that propofol and fentanyl were administrated via the ear edge vien or t! mmon carotid artery, can achieve the purpose of the maintenance of general anesthesia in rabbits. Compared with drug infusion via the ear edge vien, infusion of propofol and fentanyl via the common carotid artery is more advantageous in some aspects, such as rapid anesthesia induction and recovery, less medication, smaller impact on the hemody
