交锁髓内钉治疗股骨干骨折合并血管损伤的临床体会

摘　　要：目的：报告交锁髓内钉治疗股骨干骨折合并血管损伤的临床效果和应用体会。方法：对12例股骨干骨折合并股血管损伤的患者在修复血管的同时采用交锁髓内钉一期固定骨折。其中开放性骨折7例,闭合性骨折5例。股血管完全断裂5例,不完全断裂3例,挫伤栓塞4例。结果：所有病例患肢均成活,伤口一期愈合,无感染发生。1例患者于术后16h血栓形成,及时探查取出血栓后行大隐静脉移植修复后恢复血运。随访时间8~24个月,骨折均愈合良好,愈合时间3~7个月,平均5个月。结论：采用交锁髓内钉可以一期有效地治疗合并血管损伤的股骨干骨折。Objective：To report clinical result and experience of interlock intramedullary nail（IIN） in treating femoral shaft fracture（FSF） combined with vascular injury.Methods：Twelve patients were treated with IIN and vascular repair.Open fractures were observed in 7 cases,and close fractures in 5 cases.Femoral arteries were complete rupture in 5 cases;partly rupture in 3 cases and embolism in 4 cases.Results：All limbs survived without infection.Arterial thrombosis after surgery was observed in 1 patient,who was immediately treated with removal of the thrombus and venous graft.All patients were followed up from 8 to 24 months.The fracture achieved unions in all cases with an average duration of 5 months.Conclusion：IIN is a effective therapy for FSF combined with vascular injury.
