自体骨髓干细胞移植治疗下肢严重缺血的临床研究

摘　　要：目的：探讨自体骨髓干细胞移植治疗下肢严重缺血的临床效果。方法：对30例下肢严重缺血患者,从髂骨处抽取自体骨髓血,分离出单个核细胞,再制成细胞悬液。通过股动脉插管,在球囊阻断动脉血流的情况下将干细胞悬液注射到缺血的下肢动脉。结果：患肢疼痛、冷感及静息痛消失17例,占56.7%;减轻12例,占40.0%;无变化1例,占3.3%。间歇性跛行的行走距离延长100米的21例,占70.0%;延长50米的9例,占30.0%。足趾和（或）足部溃疡3例,完全修复1例,足部截肢2例。27例足部皮肤颜色均有不同程度恢复。ABI 0.5~0.6的15例,占50.0%;ABI 0.6~0.7的11例,占36.7%;ABI 0.7~0.8的4例,占13.3%。手术并发症3例,占10.0%;其中,发热2例,占6.7%;穿刺点血肿1例,占3.3%。结论：自体骨髓干细胞移植是治疗下肢严重缺血的一种简单、安全、有效手段。Objective：To investigate the autologous bone marrow stem cell transplantation in the treatment of severe lower limb ischemia in the clinical effects of.Methods：30 cases of severe lower limb ischemia patients,taken from the iliac of autologous bone marrow blood and isolated mononuclear cells,and then made into cell suspension.Intubation through the femoral artery,balloon occlusion in the case of arterial blood stem cells will be injected into the suspension of the lower extremity arterial ischemia.Results：Limb pain,cold feeling and the disappearance of rest pain in 17 cases,accounting for 56.7%;12 cases to reduce,accounting for 40.0%;no change in 1 case,accounting for 3.3%.Intermittent claudication walking distance of 100 meters to extend the 21 cases,accounting for 70.0%;the extension of 9 cases of 50 meters,accounting for 30.0%.Toes and（or） three cases of foot ulcers,one cases of fully restored,two cases of foot amputation.27 cases of foot skin color restoration of varying degrees.ABI 0.5～0.6 of the 15 cases,accounting for 50.0%;ABI 0.6～0.7 of the 11 cases,accounting for 36.7%;ABI 0.7～0.8 of the four cases,accounting for 13.3%.3 cases of surgical complications,accounting for 10.0%;Of them,two cases of fever,accounting for 6.7%;one cases of hematoma puncture points,or 3.3%.Conclusion：Autologous bone marrow stem cell transplantation is the treatment of severe lower limb ischemia in a simple,safe and effective means.
