摘  要:目的观察负压封闭引流(VSD)技术在不完全皮肤撕脱伤修复中的疗效,分析其优缺点,总结临床经验,提高VSD使用水平。方法选择2009年5月至2009年12月18例不完全皮肤撕脱伤患者,其中男性11例,女性7例;年龄16~58岁,平均年龄30.6岁。对可能坏死的部分修薄并制成筛状全厚或中厚皮片后原位缝合,分别采用VSD和常规加压包扎各9例,分为VSD组与常规组。术后观察可能坏死的部分皮肤成活率,数据采用SPSS13.0统计软件包分析比较,并对VSD的应用进行理论分析。结果 VSD组成活率80.78%,常规组成活率82.66%,两组比较差异无统计学意义(P>0.1);VSD早期引流充分但负压力对组织修复无益,而常规加压包扎稳定性较好,更有利于组织循环重建。结论 VSD是不完全皮肤撕脱伤修复中一种可供选择的辅助方法,但无明显的优越性,临床应用有待改进。Objective To observe the efficacy of vacuum sealing drainage(VSD) in skin incomplete avulsion and analyze its advantages,disadvantages and summarize the clinical experience for improving VSD clinical application.Methods A total of 18 cases with incomplete avulsion of skin from May 2009 to December 2009 were enrolled,male 11,female 7,aged 16-58 years,mean age 30.6 years.The part potential necrosis tissue was peeled thinner to cribriform and made the full-thickness or intermediate thickness skin graft were prepared and sutured in situ,of these patients 9 were treated by VSD,and 9 were performed by conventional compression bandage.The survival rate of possible necrosis tissue was observed,the data were analyzed by SPSS 13.0.And theoretical analysis were performed for VSD application.Results The survival rate of VSD group was 80.78 % and the conventional group was 82.66 %,there was no significant difference between 2 groups(P 0.1);The early drainage of VSD was complete,but the negative pressure was no benefit on tissue repair,while the conventional compression bandage had more stability and further beneficial to reconstruction of the blood supply.Conclusion It is demonstrated that VSD is an alternative approach in incomplete avulsion of skin,but obvious benefit is not found and the clinical application techniques should be further improved.
