摘 要:目的:探讨瑞芬太尼联合丙泊酚静脉全身麻醉用于小儿腭裂整复术的临床效果。方法:常规诱导后,将所有患儿随机分为两组,观察组插管成功后持续泵入丙泊酚1~3mg/(kg·h)、瑞芬太尼0.2~0.5μg/(kg·min)维持麻醉,对照组气管插管成功后,应用氯胺酮2~3mg/(kg·h)静脉持续泵入维持麻醉,根据患儿生命体征和手术步骤进行麻醉深度调节,手术结束后停用所有麻醉药物。记录并统计两组患儿平均动脉压(MAP)、心率(HR)变化以及两组患儿术后恢复时间比较。结果:诱导前两组患儿MAP无差别,对照组患儿插管时、插管后5min MAP高于观察组,对照组插管时、拔管时MAP值高于诱导前,观察组血压变化较平稳,诱导前两组患儿HR无差别,对照组患儿插管后5min的HR快于观察组,对照组插管时、拔管时均快于诱导前,观察组心率变化较平稳,观察组患儿在自主呼吸恢复时间、苏醒时间以及停药至拔管时间均短于对照组。结论:异丙酚复合瑞芬太尼静脉麻醉是小儿腭裂修复术的有效麻醉方法之一。Objective:To investigate the clinical effect of remifentanil and propofol general anesthesia for cleft palate repair in children. Methods:All the children were randomly divided into two groups, the observation group after successful intubation continuous infusion of propofol 1 to 3 mg/(kg·h),remifentanil 0.2 to 0.5gg/(kg·min) to maintain anesthesia in the control group after tracheal intubation, ketamine 2 to 3 mg/(kg·h) intravenous continuous infusion to maintain anesthesia, according to vital signs in children with depth of anesthesia and surgical procedure,adjusted after surgery to disable all the narcotic drugs. Two sets of records and statistics mean arterial pressure m children(MAP),heart rate(HR) change and the recovery time after surgery compared two groups of children.Results:MAP before the induction of two children had no difference in the control group patients intubation.5min MAP after intubation was higher than the observation group and control groupMAP was higher than that before, observation group blood pressure was stable,the two groups before induction of HR in children with no difference in the control group after intubation in ehildren,5min of HR was faster than the observation group and control group intubation,extubation was faster than the observation group,heart rate was stable in the observation group patients in the spontaneous breathing recovery time,recovery time.and stopping at all times was shorter than the control group.Conclusion:Propofol intravenous anesthesia with remifentanil in children with cleft palate repair is an effective anesthetic methods.
