摘 要:目的:探讨经皮桡动脉途径治疗冠心病复杂病变的可行性与安全性。方法:选择有冠心病稳定型心绞痛患者300例分为经桡动脉途径介入治疗组(n=150例,其中男96例,女54例);股动脉途径介入治疗组(n=150例,其中男100例,女50例)。分别记录PCI时间,PCI成功率,住院时间及与穿刺血管相关并发症的发生率。结果:150例患者桡动脉均穿刺成功,PCI时间为(48±3.6)分钟,PCI成功率为98.7%,住院时间为(3.2±1.6)天;股动脉组PCI时间为(45±10.2)分钟,PCI成功率为99.9%,住院时间为(5.4±1.8)天。两组相比较,PCI时时间、PCI成功率差异无统计学意义(P>0.05);桡动脉组住院时间明显缩短(P<0.05),与穿刺血管相关并发症的发生率也明显减少(P<0.001)。结论:经桡动脉途径行冠心病复杂病变PCI是一种安全、有效的介入途径。Objeetive: To assess the clinical feasibility and safety of percutaneous coronary intervention through trans-radiat approach in patients with coronary complex lesions. Methods:We retrospectively analyzed data from our single center registry on 300 patients with stable angina pectoris and underwent pereutaneous coronary intervention(PCI). All the patients were respectively divided into trans-radial group (n=150) and trans-femoral group (n=150). We recorded the PCI time, the success rate of PCI, in-hospital times, and the complication rate related to puncture vessels. Results: 150 radial arteries were punctured successfully. There were no significant differences in PCI time [(48±3.6) min vs. (45±10.2) mini and the success rates of PCI [98.7% (148/150) vs. 99.9% (149/150)] between the two groups respectively (P〉0.05). There were significant differences in-hospital times [(3.2±1.6) d vs. (5.4±1.8) d] between the two groups (P〈0.05). The access artery complications (3.3%) in TRA-PCI group were much lower than (8.0%) in TFA-PCI group(P〈0.001). Couelusions:Trans-radial approach might be selected as one approach for PCI in patients with complex lesions. It is safe and effective approach for coronary intervention in selected patients.
