精子发生阻滞不育患者睾丸病理特征

摘　　要：目的探讨精子发生阻滞睾丸病理改变的特征。方法本研究以常规病理学检查120例不育男性睾丸活检标本,按精子发生阻滞诊断标准进行诊断,并取3例精子发生阻滞于不同细胞水平的睾丸组织进行超微病理学检查。结果 120例标本中符合精子发生阻滞病理诊断标准者有31例(25.8%),其中阻滞于精原细胞水平9例(29.0%),阻滞于精母细胞水平6例(19.4%),阻滞于精子细胞水平16例(51.6%)。超微病理检查显示,精子发生阻滞不育患者睾丸支持细胞紧密连接改变、内质网和高尔基复合体变形是其特征性的结构。结论精子发生阻滞的细胞学特征明显;精子发生阻滞可能与支持细胞结构完整性和功能状态密切相关。Objective To investigate the pathological features of testis of patients with spermatogenic arrest.Methods According to the diagnostic standard of spermatogenic arrest,testicular biopsy specimens from 120 infertile men were examined by routine pathological techniques in this study.Next,three cases of testicular tissues with spermatogenic arrest at different cellular levels were examined by ultrapathological techniques.Results 31 of all the cases（26.5%）were accorded with the diagnostic standard of spermatogenic arrest.9 of 31 cases（29.0%） were at the level of spermatogonia,6 cases（19.4%）were at the level of spermatocyte,and 16 cases（51.6%）were at the level of spermatid.Ultrapathological examination showed that the characteristic structures of the testicular tissue with spermatogenic arrest were the tight junction between two sertoli cells,the endoplamic reticulum and the golgi apparatus of sertoli cells were deformed.Conclusion The cellular features of spermatogenic arrest are distinct.The spermatogenic arrest may be closely associated with structural integrity and functional status of sertoli cells.
