摘  要:[目的]比较智能监护(SmartCare)脱机模式和同步间歇指令通气+压力支持勇气(SIMV+PSV)、PSV+间断T形管通气(TC)三种脱机方法,探讨智能监护脱机模式是否可以缩短脱机时间和提高呼吸衰竭患者的脱机成功率.[方法]2008年2月至2009年8月本院ICU收治的各种因呼吸衰竭进行机械通气患者48例,随机分为SIMV+PSV组、PSV+TC组和智能监护脱机组(SC组),进行脱机治疗.记录每位患者脱机开始时的急性生理学与慢性健康状况评分系统(APACHE-Ⅱ)评分、血气分析、脱机时间.脱机成功标准为停用呼吸机48 h以上生命体征平稳,未再进行机械通气者.[结果]SC组脱机时间比SIMV+PSV组、PSV+TC组短,且差异有显著性(P〈0.05).[结论] 三种脱机方式中智能监护(SmartCare)脱机模式优于SIMV+PSV方式和PSV+TC方式.[Objective]To compare three weaning methods such as SmartCare(SC) weaning mode, SIMV +PSV mode, PSV+TC mode and to explore whether the SC weaning pattern can shorten the weaning time and enhance patient's weaning success ratio of respiratory failure. [Methods] Forty-eight cases of respiratory failure with a variety of mechanical ventilation in the ICU of our hospital from February 2008 to August 2009 were randomly divided into SIMV + PSV group, PSV + TC group and SC group for weaning treatment. Each patient's APACHE-Ⅱ grades, blood gas analysis and the weaning time were recorded. The successful weanirig standard was that life symptom was steady while mechanical ventilation was stopped for more than 48h and no mechanical ventilation was carried on again. [Results] The weaning time in SC group was shorter than that in SIMV + PSV group and PSV + TC group, and the difference had statistical significance( P 〈0. 05). [Conclusion] Among the three kinds of weaning modes, SmartCare weaning mode is better than SIMV + PSX and PSV + TC mode.
