超声引导微波消融治疗子宫肌瘤的临床研究

摘　　要：目的:探讨超声引导微波消融(MWA)治疗子宫肌瘤的有效性和安全性。方法:采用ECO-100型微波治疗仪对子宫肌瘤患者14例中共16个肌瘤进行治疗,比较治疗前后患者的超声图像和临床症状的变化,将疗效分为治愈、显效、有效和无效。结果:治疗后1、3、6、12、24个月随访,超声检查显示肌瘤体积较治疗前明显缩小(P<0.01),平均缩小率分别为(47.98±18.66)%、(57.26±16.48)%、(66.82±16.81)%、(79.87±15.04)%、(92.52±8.10)%。子宫肌瘤相关临床症状较治疗前明显改善,14例患者中治愈10例(10/14)、显效2例(2/14)、有效2例(2/14)、无效0例,总有效率为100%。1例患者治疗后出现肠穿孔。结论:MWA治疗子宫肌瘤效果显著,术后患者痛苦小、恢复快,具有良好应用前景及推广价值,值得进一步研究。Objective：To study the safety and effectiveness of ultrasound-guided microwave ablation（MWA） therapy for uterine leiomyoma.Methods：Fourteen patients with sixteen uterine leiomyomas were chosen for assessment and the mode ECO-100 of microwave therapeutic apparatus was used.The ultrasound images and clinical symptoms before and after the treatment were compared.The results were divided into cured,valid,improved and invalid.Results：The average rates of reduction in leiomyoma volume were 47.98±18.66%,57.26±16.48%,66.82±16.81%,79.87±15.04% and 92.52±8.10%,respectively after MWA 1,3,6,12 and 24 months by evaluation of ultrasound.There was a significant difference in leiomyoma volume between pre-and post-treatment（P〈0.01）.The clinical symptoms of all the patients were improved significantly after ablation.Among the patients,10 patients were cured（10/14）,2 cases were valid（2/14）,2 cases were improved（2/14） and none was invalid（0/14）.The total effective rate was 100%.One case of enterobrosis occurred after treatment.Conclusion：The treatment of uterine leiomyoma with MWA is effective and safe.The patients suffer little distress and recover soon after treatment.So the therapy is clinically prospective for application and has a popular value.
