上海市金山石化地区人群乙型肝炎血清流行病学调查

摘　　要：目的:调查金山石化地区人群乙型病毒性肝炎(乙肝)血清学感染状况。方法:2009年1月—6月6 507例在金山医院就诊人群和1 564例金山石化地区健康体检人群,用酶联免疫吸附试验(ELISA)法检测乙肝血清标志物,包括乙肝病毒表面抗原(HBsAg)、乙肝病毒表面抗体(抗-HBs)、乙肝病毒e抗原(HBeAg)、乙肝病毒e抗体(抗-HBe)和乙肝病毒核心抗体(抗-HBc和抗-HBc-IgM)。其中360例用聚合酶链反应(PCR)法进行乙肝病毒DNA定量(HBV DNA),969例用全自动生化分析仪检测血清肝功能指标,包括丙氨酸转氨酶(ALT)、天门冬氨酸转氨酶(AST)和γ谷氨酰转肽酶(GGT)。结果:就诊人群和健康体检人群的乙肝标志物血清模式分布基本一致。总体人群HBsAg阳性率10.2%,抗-HBs阳性率46.4%;健康体检人群HBsAg阳性率6.2%,抗-HBs阳性率50.9%;就诊人群HBsAg阳性率11.1%,抗-HBs阳性率45.4%。HBeAg阳性组的HBV DNA拷贝数值最高。结论:与其他地区相比,本地区HBsAg阳性率偏高。对地区人群进行乙肝情况分析,为乙肝的的诊断、治疗与免疫预防提供全面的实验依据。Objective：To survey serological infection status of hepatitis B virus（HBV） for the people in Jinshan Shihua area in Shanghai.Methods： For 6 507 Jinshan Hospital patients and 1564 healthy people from January to June in 2009,serum hepatitis B virus markers（HBsAg,anti-HBs,HBeAg,anti-HBe,anti-HBc and anti-HBc-IgM） were detected.For 360 patients,HBV DNA was also detected.For 969 patients,serum liver function markers were detected with automatic biochemistry analyzer.Results： There was the same model of hepatitis serum marker in patients and healthy people.For whole pepole,the positive rate of HBsAg was 10.2%,and the positive rate of anti-HBs was 46.4%.For healthy pepole,the positive rate of HBsAg was 6.2%,and the positive rate of anti-HBs was 50.9%;For the patients,the positive rate of HBsAg was 11.1%,and the positive rate of anti-HBs was 45.4% In the group with HBsAg＋,the copies of HBV DNA was the highest.Conclusions： The positive rate of HBsAg is higher in Jnshan Shihua than the one in the other area.The infectious analysis of hepatitis B virus for the people in Jinshan Shihua area,which provides the comprehensive experimental basis for the diagnosis,treatment and immune preventive of HBV.
